快速康复外科护理对剖宫产术后产妇早期离床活动的影响

Effect of Enhanced Recovery After Surgery Nursing on Early Ambulation of Parturients After Cesarean Section

目的 研究快速康复外科护理对剖宫产术后产妇早期离床活动的影响。方法 选取2019年1月-2021年6月峡江县人民医院行剖宫产分娩的80例产妇,采用随机数字表法分为对照组(40例)和观察组(40例)。对照组给予常规护理,观察组应用快速康复外科护理,比较两组术后恢复时间(肠鸣音恢复时间、首次排便时间、术后住院时间)、术后24 h疼痛情况[视觉模拟评分(VAS)]、早期离床活动情况(首次离床活动时间、术后24 h离床率)、术后并发症、母乳喂养情况(泌乳始动时间、产后3天的泌乳充足率、纯母乳喂养率)。结果 观察组术后恢复时间(肠鸣音恢复时间、首次排便时间、术后住院时间)短于对照组,且术后24 h VAS评分小于对照组(P<0.05);观察组首次离床活动时间短于对照组,且术后24 h离床率高于对照组(P<0.05);观察组术后并发症发生率小于对照组(P<0.05);观察组泌乳始动时间短于对照组,且泌乳充足率、纯母乳喂养率高于对照组(P<0.05)。结论 快速康复外科护理可缩短剖宫产产妇术后恢复时间,缓解其术后疼痛,促进产妇早期离床活动,降低其并发症风险,改善母乳喂养情况。

Objective To study the effect of enhanced recovery after surgery nursing on early ambulation of parturients after cesarean section.Methods A total of 80 parturients who underwent cesarean section in Xiajiang County People's Hospital from January 2019 to June 2021 were selected and divided into control group(40 parturients)and observation group(40 parturients)by random number table method.The control group was given routine nursing,and the observation group was given enhanced recovery after surgery nursing.The postoperative recovery time(bowel sound recovery time,first defecation time,postoperative hospital stay),pain at 24 hours after operation[Visual Analogue Scale(VAS)],early off-bed activity(first off-bed activity time,off-bed rate at 24 hours after operation),postoperative complications,breastfeeding(lactation initiation time,lactation adequacy rate at 3 days after delivery,exclusive breastfeeding rate)were compared between the two groups.Results The postoperative recovery time(bowel sound recovery time,first defecation time,postoperative hospital stay)in the observation group was shorter than that in the control group,and the VAS score at 24 h after operation was lower than that in the control group(P<0.05).The first time of leaving bed in the observation group was shorter than that in the control group,and the rate of leaving bed at 24 h after operation was higher than that in the control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).The starting time of lactation in the observation group was shorter than that in the control group,and the lactation adequacy rate and exclusive breastfeeding rate were higher than those in the control group(P<0.05).Conclusion Enhanced recovery after surgery nursing can shorten the recovery time after cesarean section,relieve postoperative pain,promote early ambulation,reduce the risk of complications and improve breastfeeding.
