sPLA2R-Ab滴度在特发性膜性肾病中的相关性研究

Correlation Study of sPLA2R-Ab Titer in Idiopathic Membranous Nephropathy

目的 探讨sPLA2R-Ab滴度与特发性膜性肾病(IMN)患者临床相关性,以期为临床诊断及治疗提供依据。方法 选取2021年1月至2023年1月,潍坊市人民医院肾内科诊断为IMN的91例患者作为研究对象,根据患者sPLA2R-Ab滴度不同分为阴性组、低浓度组和高浓度组,同时随机选取65例非膜性肾病(NO-IMN)患者作为NO-IMN组。分析不同滴度组间的病理分期,以及各组之间血清学指标和尿液学指标的关系。结果 IMN组中sPLA2R-Ab阳性患者70例,阳性率76.92%,48例患者处于低浓度组中。IMN组的24 h蛋白尿(24hUTP)、尿微量白蛋白(UALB)、白蛋白(ALB)、三酰甘油(TG)、总胆固醇(TC)和sPLA2R-Ab与NO-IMN组的差异有统计学意义(P<0.05)。不同滴度组间血清学指标ALB、LDL、sPLA2R-Ab和全血Total-B(CD19^(+))差异有统计学意义(P<0.05),尿液学指标24hUTP,UALB,UIgG的差异有统计学意义(P<0.05)。结论 sPLA2R-Ab滴度可作为IMN诊断的重要血清学检测指标,s PLA2R-Ab滴度还与IMN的病理分期以及临床症状相关,可根据sPLA2R-Ab滴度制订临床治疗方案。

Objective To explore the clinical correlation between sPLA2R-Ab titer and IMN patients,in order to provide evidence for clinical diagnosis and treatment.Methods Ninety-one patients diagnosed with idiopathic membranous nephropathy(IMN)in the Department of Renal Medicine at Weifang People's Hospital between January 2021 and January 2023 were selected as the study subjects.Based on the different levels of sPLA2R-Ab titers,the patients were categorized into a negative group,low concentration group,and high concentration group.Additionally,65 patients with non-membrane nephrotic diseases(NO-IMN)were randomly chosen as the NO-IMN group.The pathological stages of different titer groups and the relationship between serological and urological indexes were analyzed.Results In the IMN group,70 patients were positive for sPLA2R-Ab,with a positive rate of 76.92%,and 48 patients were in the low concentration group.The differences of 24hUTP,UALB,ALB,TG,TC and sPLA2R-Ab in IMN group were statistically significant compared with NO-IMN group(P<0.05).There were significant differences in serum indices ALB,LDL,sPLA2R-Ab and Total blood-B(CD19^(+))among different titers(P<0.05),there were statistically significant differences in urinological indexes of 24hUTP,UALB and UIgG(P<0.05).Conclusions sPLA2R-Ab titer can be used as an important serological indicator for the diagnosis of IMN.sPLA2R-Ab titer is also related to the pathological stage and clinical symptoms of IMN,and clinical treatment plan can be formulated according to sPLA2R-Ab titer.
