摘要
目的 探讨磁共振弥散加权成像(DWI)联合磁共振血管成像(MRA)对急性脑梗死患者的诊断价值。方法 本研究于2021年5月至2022年5月,以梁山县人民医院收治的100例急性缺血性脑卒中(ACI)患者为研究对象,对MRI技术在脑梗死诊断中的临床应用进行研究。对100例ACI患者的病灶分布进行统计并计算DWI、MRA单独及组合诊断ACI的效果。结果 100例ACI患者中单发病灶、多发病灶各有69例、31例;100例ACI患者中DWI检测阳性93例,阳性诊断率为93.00%(93/100),且表现为高信号病灶;发病时间≤6 h内患者45例,DWI检测阳性45例,阳性诊断率为100.00%(45/45);发病时间6~24 h患者40例,DWI检测阳性35例,阳性诊断率为87.50%(35/40);发病时间24~74 h患者15例,DWI检测阳性14例,阳性诊断率为93.33%(14/15)。100例ACI患者中MRA检测阳性90例,阳性诊断率为90.00%(90/100);DWI与MRA联合检测的阳性诊断率稍高于单独DWI或MRA检测,且这3种检测的阳性诊断率并无差别(P>0.05)。结论 通过采用DWI技术,可以敏感地检测脑组织中的梗死灶弥散变化,为早期脑梗死的诊断提供了有力支持,MRA技术可以直观地观察脑血管结构,提供了关键信息,有助于了解梗死灶周围的血流情况。这两者的联合应用使得诊断更为全面、准确。
Objective To explore the diagnostic application of diffusion-weighted magnetic resonance imaging combined with magnetic resonance angiography in patients with acute cerebral infarction.Methods This study was conducted from May 2021 to May 2022,with 100 patients with acute ischemic stroke(ACI)admitted to Liangshan County People's Hospital as the research subjects.This study demonstrated the comprehensive application of MRI technology in the diagnosis of cerebral infarction by detecting and processing images of 100 patients with cerebral infarction(ACI)using magnetic resonance scanners.Perform statistical analysis on the distribution of lesions in 100 ACI patients.Calculate the effectiveness of DWI and MRA alone and in combination in diagnosing ACI.Results Among 100 ACI patients,there were 69 cases of single lesion and 31 cases of multiple lesions,respectively;Among 100 ACI patients,93 cases were positive for DWI detection,with a positive diagnostic rate of 93.00%(93/100),and showed high signal lesions;Within≤6 hours of onset,there were 45 patients with positive DWI detection,with a positive diagnostic rate of 100.00%(45/45);There were 40 patients with a onset time of 6-24 hours,and 35 of them tested positive for DWI,with a positive diagnostic rate of 87.50%(35/40);There were 15 patients with a onset time of 24-74 hours,and 14 of them tested positive for DWI,with a positive diagnostic rate of 93.33%(14/15).Out of 100 ACI patients,90 cases tested positive for MRA,with a positive diagnostic rate of 90.00%(90/100);The positive diagnostic rate of the combined detection of DWI and MRA is slightly higher than that of DWI or MRA alone,however,there is no significant difference in the positive diagnostic rate of these three tests(P>0.05).Conclusions By using DWI technology,the diffusion changes of infarcted lesions in brain tissue can be sensitively detected,providing strong support for the diagnosis of early cerebral infarction.MRA technology can intuitively observe the structure of cerebral blood vessels,provide key information,and help understand the blood flow situation around the infarcted lesion.The combined application of these two makes diagnosis more comprehensive and accurate..
