高剂量异烟肼对药物敏感复治肺结核患者肺功能及免疫功能的影响

摘要 目的:观察高剂量异烟肼对药物敏感复治肺结核患者肺功能及免疫功能的影响。方法:选取2022年1月—2023年1月贵阳市公共卫生救治中心收治的药物敏感复治肺结核患者60例作为研究对象,依据随机数字表法分为对照组与研究组,各30例。对照组接受标准剂量异烟肼方案,研究组接受高剂量异烟肼方案。比较两组肺功能及免疫指标水平。结果:治疗前,两组最大呼气流量(PEF)、第1秒用力呼气容积(FEV_(1))比较,差异无统计学意义(P>0.05);治疗后,两组PEF、FEV_(1)升高,研究组高于对照组,差异有统计学意义(P<0.05)。治疗前,两组CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平比较,差异无统计学意义(P>0.05);治疗后,两组CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)水平升高,研究组高于对照组,差异有统计学意义(P<0.05);两组CD8^(+)水平降低,研究组低于对照组,差异有统计学意义(P<0.05)。结论:高剂量异烟肼有利于促进药物敏感复治肺结核患者肺功能恢复,并能提高其免疫功能。 Objective:To observe the effect of high dose isoniazid on pulmonary function and immune function in patients with drug-sensitive retreatment of pulmonary tuberculosis.Methods:A total of 60 patients with drug-sensitive pulmonary tuberculosis were selected as the research objects at Guiyang Public Health Treatment Center from January 2022 to January 2023,and were divided into control group and study group according to random number table method,with 30 cases in each group.The control group received the standard dose isoniazid regimen,and the study group received the high dose isoniazid regimen.The levels of lung function and immune indexes were compared between the two groups.Results:Before treatment,there was no significant difference in maximum expiratory flow(PEF)and forced expiratory volume in the first second(FEV_(1))between the two groups(P>0.05).After treatment,PEF and FEV_(1) increased in two groups,and the study group was higher than the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the levels of CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)between the two groups(P>0.05).After treatment,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in two groups were increased,and the study group was higher than the control group,the difference was statistically significant(P<0.05).The level of CD8^(+)in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:High dose of isoniazid can promote the recovery of lung function and improve the immune function of patients with pulmonary tuberculosis after drug sensitive retreatment.
