低分子肝素对高危肺癌患者术后发生静脉血栓栓塞的预防价值

Preventive value of low molecular weight heparin for postoperative venous thromboembolism in high-risk lung cancer patients

目的 探讨低分子肝素对高危肺癌患者术后发生静脉血栓栓塞(VTE)的预防价值。方法 收集2021年5月至2023年6月于首都医科大学附属北京朝阳医院接受手术治疗的237例肺癌患者的临床资料,按照术后是否使用低分子肝素将患者分为干预组(n=129)和对照组(n=108)。比较两组患者的专科信息、手术信息以及术后住院期间VTE相关结局。结果 两组患者的组织学分型、TNM分期、手术时间和术中失血情况比较,差异均无统计学意义(P﹥0.05)。干预组患者的术后VTE发生率、术后D-二聚体水平均低于对照组患者,差异均有统计学意义(P﹤0.05)。两组患者术后轻微出血的比例、凝血酶原时间及活化部分凝血活酶时间比较,差异均无统计学意义(P﹥0.05)。结论 低分子肝素可有效、安全地降低高危肺癌患者术后发生VTE的风险,值得临床推广。

Objective To investigate the preventive value of low molecular weight heparin for postoperative venous thromboembolism(VTE)in high-risk lung cancer patients.Method The clinical data of 237 lung cancer patients who underwent surgical treatment in the Beijing Chao-yang Hospital,Capital Medical University from May 2021 to June 2023 were collected and divided into intervention group of 129 cases and control group of 108 cases based on whether low molecular weight heparin was used after surgery.The specialized information,surgical information,and VTE related outcomes during postoperative hospitalization between two groups of patients were compared.Result There were no statistically significant differences in the histological classification,TNM staging,surgical time,and intraoperative blood loss between the two groups of patients(P>0.05).The incidence of postoperative VTE and D-dimer levels in the intervention group were lower than those in the control group,and the differences were statistically significant(P<0.05).There were no statistically significant differences in the proportion of postoperative minor bleeding,prothrombin time,and activated partial thromboplastin time between the two groups of patients(P>0.05).Conclusion Low molecular weight heparin can effectively and safely reduce the risk of postoperative VTE in high-risk lung cancer patients,and is worthy of clinical utility.
