间歇性气压联合踝泵运动对老年急性缺血性脑卒中患者住院期间下肢深静脉血栓的预防价值

Preventive value of intermittent barometric pressure combined with ankle pump exercise for lower extremity deep vein thrombosis in elderly patients with acute ischemic stroke during hospitalization

目的 探讨间歇性气压联合踝泵运动对老年急性缺血性脑卒中患者住院期间下肢深静脉血栓的预防价值。方法 收集2020年2月至2022年12月首都医科大学附属北京友谊医院收治的126例老年急性缺血性脑卒中患者的临床资料,根据住院期间是否接受间歇性气压联合踝泵运动治疗分为联合组(n=57)和对照组(n=69)。比较两组患者住院期间下肢深静脉血栓发生率、D-二聚体水平、凝血功能指标以及住院期间并发症发生率。结果 住院期间共发生下肢深静脉血栓12例,联合组患者下肢深静脉血栓发生率低于对照组患者,差异有统计学意义(P﹤0.05)。联合组患者出院前D-二聚体水平低于对照组患者,差异有统计学意义(P﹤0.05)。两组患者出院前凝血功能指标及并发症发生率比较,差异均无统计学意义(P﹥0.05)。结论 间歇性气压联合踝泵运动对老年急性缺血性脑卒中患者住院期间下肢深静脉血栓有较高的预防价值和安全性,值得临床推广。

Objective To investigate the preventive value of intermittent barometric pressure combined with ankle pump exercise on deep venous thrombosis of lower extremity in elderly patients with acute ischemic stroke during hospitalization.Method Clinical data of 126 elderly patients with acute ischemic stroke admitted to Beijing Friendship Hospital,Capital Medical University from February 2020 to December 2022 were collected.They were divided into combination group(n=57)and control group(n=69)according to whether they received intermittent air pressure combined with ankle pump exercise during hospitalization.The incidence of lower extremity deep vein thrombosis,D-dimer level,coagulation function index and complication rate during hospitalization were compared between the two groups.Result There were 12 cases of lower extremity deep vein thrombosis during hospitalization,the incidence rate of lower extremity deep vein thrombosis in combination group was lower than that in control group,the difference was statistically significant(P<0.05).The level of D-dimer in combination group was lower than that in control group before discharge,the difference was statistically significant(P<0.05).There was no significant difference in coagulation function index and complication rate between two groups before discharge(P>0.05).Conclusion Intermittent air pressure combined with ankle pump exercise has high preventive value and safety for lower extremity deep vein thrombosis in elderly patients with acute ischemic stroke during hospitalization,which is worthy of clinical promotion.
