喹硫平联合丙戊酸镁治疗对双相情感障碍躁狂发作患者内分泌激素、认知功能的影响

Effects of quetiapine combined with magnesium valproate on endocrine hormone and cognitive function in patients with manic episode of bipolar disorder

目的:分析喹硫平联合丙戊酸镁治疗对双相情感障碍躁狂发作患者的影响。方法:选取2022年7月至2023年1月我院收治的64例双相情感障碍躁狂发作患者,采用随机数字表法分为研究组及对照组,每组各32例。对照组接受丙戊酸镁治疗,研究组接受喹硫平联合丙戊酸镁治疗,两组均治疗8w。治疗8w后评估疗效,治疗前、治疗8 w后采用连线测验包括连线测试A(Trail making testA,TMTA)及连线测试B(Trail making test b,TMTB)、威斯康星卡片分类测验(Wsiconsin card sorting test,WCST)、言语记忆测验(Hopkins verbal learning test,HVLT-R)评估患者认知功能,通过化学发光免疫分析法检测血清泌乳素(Prolactin,PRL)、促甲状腺激素(Thyroid stimulating hormone,TSH)、游离甲状腺素(Freethyroxine4,FT4)、游离三碘甲状腺原氨酸(Free thyroxine3,FT3)水平。结果:治疗后,研究组的治疗总有效率显著高于对照组(P<0.05)。治疗后,两组TMTA、TMTB用时缩短,且研究组更短(P<0.05)。与治疗前比较,治疗8 w后两组血清PRL、TSH水平升高,血清FT4、FT3水平均较治疗前降低(P<0.05),但组间比较差异无统计学意义(P>0.05)。结论:喹硫平联合丙戊酸镁治疗双相情感障碍躁狂发作患者的效果良好,可改善认知功能,并未增加治疗对患者内分泌激素的影响。

Objective:To analyze the effect of quetiapine combined with magnesium valproate on manic episode patients with bipolar disorder.Methods:64 patients with bipolar manic episode admitted to our hospital from July 2022 to January 2023 were selected and divided into study group and control group by random number table method,with 32 cases in each group.The control group received magnesium valproate treatment,and the study group received quetiapine combined with magnesium valproate treatment,both groups were treated for 8 weeks.The efficacy was evaluated after 8 weeks of treatment,and connection tests were used before and after 8 weeks of treatment,including Trail making test A(TMTA)and Trail making test B(TMTB),Wsiconsin card sorting test(WCST),and Hopkins verbal learning test(HVLT-R)assessed cognitive function.Serum Prolactin(PRL),Thyroid stimulating hormone(TSH)and Free thyroxine4(FT4)and Free thyroxine 3(FT3)levels were detected by chemiluminescence immunoassay.Results:After treatment,the total effective rate of the study group was significantly higher than that of the control group(P<0.05).After treatment,the time of TMTA and TMTB was shortened in the two groups,and shorter in the study group(P<0.05).Compared with before treatment,serum PRL and TSH levels increased and serum FT4 and FT3 levels decreased after 8 w of treatment(P<0.05),but there was no statistical significance between groups(P>0.05).Conclusion:Quetiapine combined with magnesium valproate has a good effect in the treatment of bipolar manic episode patients,which can improve cognitive function and does not increase the effect of treatment on endocrine hormones.
