E-cad、CK14、Apollon表达与食管癌患者病理特征的关系研究

摘要 目的:探讨食管癌(Esophageal Cancer,EC)患者E-钙黏蛋白(E-cadherin,E-cad)、细胞角蛋白14(Cytokeratin 14,CK14)、凋亡抑制蛋白Apollon(Inhibitor of Apoptosis Protein Apollon,IAP Apollon)表达及其与病理参数的相关性。方法:选取我院2018年1月至2020年1月期间收治的87例EC患者作为研究对象。分别取患者EC组织和癌旁组织。采用免疫组化法检测E-cad、CK14、Apollon在EC组织和癌旁组织中的表达。分析比较不同病理学参数EC组织中E-cad、CK14、Apollon的表达情况。分析EC组织中E-cad、CK14、Apollon表达与临床病理学参数相关性。探讨E-cad、CK14、Apollon表达与EC患者预后关系。结果:EC组织中CK14、Apollon阳性表达率显著高于癌旁组织,Ⅰ-Ⅱ期显著低于Ⅲ-Ⅳ期,高分化<中分化<低分化,无淋巴结转移显著低于有淋巴结转移(P<0.05)。EC组织中E-cad阳性表达率显著低于癌旁组织,Ⅰ-Ⅱ期显著高于Ⅲ-Ⅳ期,高分化>中分化>低分化,无淋巴结转移显著高于有淋巴结转移(P<0.05)。EC组织中E-cad阳性表达率与肿瘤转移(Tumor Node Metastasis,TNM)分期、淋巴结转移呈负相关,与分化程度呈正相关。CK14、Apollon阳性表达率与TNM分期、淋巴结转移呈正相关,与分化程度呈负相关(P<0.05)。E-cad阳性EC患者3年生存率高于E-cad阴性患者,CK14阳性、Apollon阳性EC患者3年生存率分别低于CK14阴性、Apollon阴性患者(P<0.05)。结论:EC患者癌组织中CK14、Apollon呈现高表达,E-cad呈现低表达,与EC发生、发展、分化程度、TNM分期、淋巴转移及预后有紧密联系。 Objective:Study on Esophageal Cancer(EC)patients with E-cadherin(E-cad),Cytokeratin 14(CK14),IAP Apollon(Inhibitor of Apoptosis Protein Apollon)expression and its correlation with pathological parameters.Methods:A total of 87 patients with EC admitted to our hospital from January 2018 to January 2020 were selected as the study objects.EC tissue and paracancer tissue were taken respectively.The expression of E-cad,CK14 and Apollon in EC and para-cancerous tissues was detected by immunohistochemistry.The expression of E-cad,CK14 and Apollon in EC tissues with different pathological parameters were analyzed and compared.The correlation between the expression of E-cad,CK14,Apollon and clinicopathological parameters in EC tissues was analyzed.To investigate the relationship between expression of E-cad,CK14,Apollon and prognosis of EC patients.Results:The positive expression rates of CK14 and Apollon in EC tissues were significantly higher than those in para-carcinoma tissues,and those in stages I-ⅱwere significantly lower than those in stages III-ⅳ,with high differentiation<medium differentiation<low differentiation,and no lymph node metastasis was significantly lower than that with lymph node metastasis(P<0.05).The positive expression rate of E-cad in EC tissues was significantly lower than that in paracancer tissues,and that inⅠ-Ⅱstages was significantly higher than that inⅢ-Ⅳstages,highly differentiated>moderately differentiated>poorly differentiated,and no lymph node metastasis was significantly higher than that with lymph node metastasis(P<0.05).The positive expression rate of E-cad in EC tissues was negatively correlated with TNM stage and lymph node metastasis,but positively correlated with differentiation degree.CK14 and Apollon positive expression rates were positively correlated with TNM stage and lymph node metastasis,but negatively correlated with differentiation degree(P<0.05).The 3-year survival rate of E-cad positive EC patients was higher than that of E-cad negative patients,and the 3-year survival rate of CK14 positive and Apollon positive EC patients was lower than that of CK14 negative and Apollon negative patients,respectively(P<0.05).Conclusion:The expression of CK14 and Apollon is high and the expression of E-cad is low,which is closely related to the occurrence,development,differentiation degree,TNM stage,lymphatic metastasis and prognosis of EC patients.
