中药外敷熏洗辅助玻璃酸钠注射液治疗退变性膝关节炎效果分析

Effect analysis of fumigating and washing with Chinese medicine and sodium hyaluronate injection in the treatment of degenerative knee arthritis

目的:研究中药外敷熏洗联合玻璃酸钠注射液对退变性膝关节炎(Osteowrtbntis of knee;OA)的临床疗效。方法:选取2022年1月至2023年1月于我院确诊的150例OA患者作为研究对象,以随机数字表法分为研究组和参照组,各75例。参照组采用玻璃酸钠注射液治疗,研究组在参照组的基础上增加中药外敷熏洗治疗。比较两组临床疗效、治疗前后中医证候积分、关节功能指标及疼痛程度、炎症因子指标和不良反应。结果:研究组临床总有效率明显高于参照组(P<0.05)。治疗后,研究组中医证候积分、关节炎指数(Western Ontario and McMaster Universities Osteoarthritis Index,WOMAC)评分、视觉模拟评分(Visual Analogue Scale;VAS)评分、胰岛素样生长因子1(Insulin like growth factor-1,IGF-1)、白细胞介素-8(Interleukin-8,IL-8)、超敏C反应蛋白(High sensitivity C-reactive protein,hs-CRP)、白细胞介素-1β(Interleukin-8,IL-1β)水平明显低于参照组(P<0.05);研究组关节活动范围(Range of motion,ROM)评分明显高于参照组(P<0.05)。两组不良反应总发生率无明显差异(P>0.05)。结论:中药外敷熏洗联合玻璃酸钠注射液治疗OA效果明显,可改善关节功能,减轻疼痛程度,降低炎症因子水平,促进病情转归,且安全性较高。

Objective:To study the clinical effect of fumigating and washing with traditional Chinese medicine combined with sodium hyaluronate injection on Osteowrtbntis of knee(OA).Methods:A total of 150 cases of OA diagnosed in our hospital from January 2022 to January 2023 were selected as research objects,and were divided into study group and reference group by random number table method,with 75 cases in each group.The reference group was treated with sodium hyaluronate injection,and the study group was treated with fumigating and washing of traditional Chinese medicine based on the reference group.The clinical efficacy,TCM syndrome score,joint function index and pain degree,inflammatory factor index and adverse reaction were compared between the two groups.Results:The total clinical effective rate of the study group was significantly higher than that of the reference group(P<0.05).After treatment,the levels of TCM syndrome score,Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score,visual analogue score(VAS)score,insulin-like growth factor 1(IGF-1),interleukin-8(IL-8),High sensitivity C-reactive protein(hs-CRP),interleukin-1β(IL-1β)in study group were significantly lower than those in control group(P<0.05).The joint range of motion(ROM)score of the study group was significantly higher than that of the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of fumigating and washing of traditional Chinese medicine with sodium hyaluronate injection in the treatment of OA has obvious effect,which can improve joint function,reduce pain degree,reduce inflammatory factor level,promote the outcome of the disease,and has high safety.
