IL-6、Hcy及TG/HDL-C联合检测对糖尿病患者周围神经病变的预测价值

Predictive value of combined detection of IL⁃6,Hcy,and TG/HDL⁃C in diabetic patients with peripheral neuropathy

摘要 目的分析研究白细胞介素6(IL-6)、同型半胱氨酸(Hcy)及三酰甘油/高密度脂蛋白胆固醇比值(TG/HDL-C)联合检测对糖尿病患者周围神经病变的预测价值。方法选取2020年1月至2023年6月阜南县人民医院收治102例的糖尿病患者作为研究对象,将并发周围神经病变的患者分为并发组(n=38),无并发周围神经病变的患者为对照组(n=64)。采用单因素、多因素Logistic回归分析糖尿病患者并发周围神经病变的影响因素,并绘制ROC曲线分析IL-6、Hcy与TG/HDL-C对糖尿病患者并发周围神经病变的预测价值。结果单因素分析结果显示,合并其他并发症及IL-6、Hcy、TG、TG/HDL-C水平均是糖尿病患者并发周围神经病变的影响因素(P<0.05);多因素Logistic回归分析显示,有合并其他并发症(OR=1.872)、IL-6水平升高(OR=2.284)、Hcy水平升高(OR=2.100)、TG水平升高(OR=2.151)及TG/HDL-C升高(OR=2.083)均是糖尿病患者并发周围神经病变的独立危险因素(P<0.05);ROC曲线分析显示,IL-6、Hcy、TG/HDL-C水平及三者联合检测评估糖尿病患者并发周围神经病变的曲线下面积(AUC)分别为0.856、0.845、0.799、0.948,联合检测优于单一检测(P<0.05)。结论IL-6、Hcy、TG/HDL-C在糖尿病周围神经病变患者中呈高表达,可能成为评估糖尿病患者并发周围神经病变的辅助预测指标。 Objective To analyze the predictive value of combined detection of interleukin⁃6(IL⁃6),homocysteine(Hcy)and triglyceride/high⁃density lipoprotein cholesterol ratio(TG/HDL⁃C)for peripheral neuropathy in diabetic patients.Methods A total of 102 diabetic patients admitted to Funan County Peoples Hospital from January 2020 to June 2023 were selected as the research subjects.The patients with peripheral neu⁃ropathy were divided into the complication group(n=38),while the patients without peripheral neuropathy were placed in the control group(n=64).Univariate and multivariate logistic regression were used to analyze the influencing factors of peripheral neuropathy in diabetic patients.Additionally,an ROC curve was drawn to analyze the predictive value of IL⁃6,Hcy and TG/HDL⁃C in diabetic patients with peripheral neuropathy.Results Univariate analysis showed that the combination of other complications,IL⁃6,Hcy,TG,and TG/HDL⁃C levels were influencing factors for peripheral neuropathy in diabetic patients(P<0.05).Multivariate lo⁃gistic regression analysis revealed that merged other complications(OR=1.872),elevated IL⁃6 level(OR=2.284),elevated Hcy level(OR=2.100),elevated TG level(OR=2.151),and elevated TG/HDL⁃C ratio(OR=2.083)were independent risk factors for peripheral neuropathy in diabetic patients(P<0.05).ROC curve analy⁃sis indicated that the area under the curve(AUC)for IL⁃6,Hcy,TG/HDL⁃C and combined detection in evaluat⁃ing peripheral neuropathy in diabetic patients were 0.856,0.845,0.799 and 0.948,respectively.Moreover,combined detection was found to be superior to single detection(P<0.05).Conclusion IL⁃6,Hcy and TG/HDL⁃C are highly expressed in patients with diabetic peripheral neuropathy.This may serve as an additional pre⁃dictor for evaluating diabetic patients with peripheral neuropathy.
