摘要
目的探讨长链非编码RNA(lncRNA)MEG3和miR-21在系统性红斑狼疮(SLE)患者外周血中的表达水平及临床意义。方法选取2020-2021年于湖北医药学院附属人民医院就诊的50例新发活动期SLE患者作为SLE组,另选取同期50例体检的健康人群作为对照组。逆转录PCR法检测两组外周血lncRNA MEG3和miR-21表达差异,比色法和透射法检测血清补体C3、C4和免疫球蛋白(Ig)G、IgA、IgM,间接免疫荧光法检测抗ds-DNA抗体,ELISA法检测白细胞介素(IL)-1、IL-6、IL-8、肿瘤坏死因子-α(TNF-α)和超敏-C反应蛋白(hs-CRP)水平,采用Pearson和Spearman相关性分析评估lncRNA MEG3和miR-21与临床指标及炎性因子的相关性,受试者工作特征(ROC)曲线评估lncRNA MEG3和miR-21对SLE的诊断效能。结果与对照组比较,SLE组补体C3、C4、lncRNA MEG3水平更低,IgG、IgA、IL-1、IL-6、IL-8、TNF-α、hs-CRP、miR-21水平及抗dsDNA阳性率更高(P<0.05)。SLE组lncRNA MEG3表达与IgG、IgA、miR-21、IL-1、IL-6、IL-8、TNF-α、hs-CRP呈负相关,与补体C3、C4呈正相关(P<0.05);miR-21与IgG、IgA、IL-1、IL-6、IL-8、TNF-α、hs-CRP呈正相关,与补体C3、C4呈负相关(P<0.05)。ROC曲线结果显示,lncRNA MEG3联合miR-21诊断SLE的曲线下面积为0.958(95%CI:0.926~0.990),灵敏度、特异度、Youden指数分别为92.0%、86.0%、0.780。结论lncRNA MEG3在SLE患者中表达水平降低,miR-21表达水平升高,二者联合对SLE有一定的诊断价值。
Objective To investigate the expression level of long chain non-coding RNA MEG3(lncRNA MEG3)and miR-21 in peripheral blood of patients with systemic lupus erythematosus(SLE)and their clinical significance.Methods A total of 50 patients with newly active SLE in the People’s Hospital Affiliated to Hubei University of Medicine from 2020 to 2021 were selected as the SLE group,and another 50 healthy people were selected as the control group.Reverse transcription polymerase chain reaction(PCR)was used to detect the difference of expression of lncRNA MEG3 and miR-21 in peripheral blood between the two groups.Serum complement C3 and C4,immunoglobulin(Ig)G,IgA and IgM were detected by colorimetric method and transmission method,and anti-ds-DNA antibody was detected by indirect immunofluorescence method.The levels of interleukin(IL)-1,IL-6,IL-8,tumor necrosis factor-α(TNF-α)and high-sensitivity C-reactive protein(hs-CRP)were detected by ELISA.Pearson and Spearman correlation analysis were used to evaluate the correlation between lncRNA MEG3 and miR-21 and clinical indicators and inflammatory factors,and the diagnostic efficacy of lncRNA MEG3 and miR-21 in SLE was evaluated by the receiver operating characteristics(ROC)curve.Results Compared with the control group,the levels of complement C3,C4 and lncRNA MEG3 in the SLE group were lower,while the levels of IgG,IgA,IL-1,IL-6,IL-8,TNF-α,hs-CRP and miR-21 and the positive rate of anti-ds-DNA were higher(P<0.05).The expression of lncRNA MEG3 in the SLE group was negatively correlated with IgG,IgA,miR-21,IL-1,IL-6,IL-8,TNF-αand hs-CRP,and positively correlated with complement C3 and C4(P<0.05).MiR-21 was positively correlated with IgG,IgA,IL-1,IL-6,IL-8,TNF-αand hs-CRP,and negatively correlated with C3 and C4(P<0.05).The ROC curve showed that the area under the curve of lncRNA MEG3 combined with miR-21 was 0.958(95%CI:0.926-0.990),and the sensitivity,specificity and Youden index were 92.0%,86.0%and 0.780,respectively.Conclusion The expression level of lncRNA MEG3 in SLE patients is decreased,and the expression level of miR-21 is increased.The combination of lncRNA MEG3 and miR-21 has certain diagnostic value for SLE.
