胃充盈超声检查在胃神经鞘瘤诊断中的价值

The value of gastric filling ultrasonography in the diagnosis of gastric schwannoma

目的 探讨胃充盈超声检查(GFUS)在胃神经鞘瘤(GS)诊断及鉴别诊断中的价值。方法 选取16例经手术病理及免疫组化证实的GS患者临床资料,术前均行GFUS,对照术后病理及免疫组化结果,分析GFUS在GS诊断及治疗中的应用价值。结果 16例GS均为单发,位于胃体10例,胃窦部3例,胃窦体交界处3例;均来源于固有肌层,表现为边界清晰低回声结节,内部均未见坏死囊变,3例内部存在钙化;4例肿瘤表面可见溃疡形成;6例有胃周淋巴结肿大;彩色多普勒超声检查10例病灶内可见血流信号。结论 GS具有特征性声像图表现,GFUS方便易行,有助于GS早期发现、诊断及术前评估。

Objective To evaluate the value of gastric filling ultrasonography in the diagnosis and differential diagnosis of gastric schwannoma.Methods The clinical data of 16 patients with GS confirmed by pathology and immunohistochemistry were analyzed retrospectively.All patients underwent GFUS before operation.Compared with the postoperative pathological and immunohistochemical results,the application value of GFUS in the diagnosis and treatment of GS was analyzed.Results GS was single in all 16 cases.10 cases located in gastric body,3 cases located in gastric antrum,and 3 cases located in antrum junc⁃tion.All cases originated from muscularis propria of stomach and showed hypoechoic nodules with clear boundary.There was no internal necrotic cyst in all cases,3 cases presented with internal calcification.Ulceration in the tumor’s mucous surface was found in 4 cases.Enlarged perigastric lymph nodes were found in 6 cases,and 10 cases showed flow signals within the lesions by color Doppler flowing imaging.Conclusions GS has characteristic sonographic findings,and GFUS is convenient,which is helpful for early detection,diagnosis and preoperative evaluation of GS.
