中药熏洗用于关节镜下关节清理术联合关节腔注射富血小板血浆治疗膝关节骨性关节炎中的效果及对TLR-4、IL-1β表达的影响

Effect of traditional Chinese medicine fumigation and washing on arthroscopic joint debridement combined with intra-articular injection of platelet-rich plasma in the treatment of knee osteoarthritis and its influences on the expression of TLR-4 and IL-1β

目的探究中药熏洗用于关节镜下关节清理术联合关节腔注射富血小板血浆治疗膝关节骨性关节炎(KOA)中的效果及对Toll样受体-4(TLR-4)、白细胞介素-1β(IL-1β)表达的影响。方法选择2021年1月至2022年7月收治的50例KOA患者为研究对象,以随机数字表法将其分为对照组和观察组,每组25例。对照组接受关节镜下关节清理术联合关节腔注射富血小板血浆治疗,观察组在对照组基础上加施中药熏洗。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的TLR-4、IL-1β水平低于对照组(P<0.05)。治疗后,观察组的西安大略麦克马斯特大学骨关节炎指数可视化量表(WOMAC)评分低于对照组,Lysholm膝关节评分高于对照组(P<0.05)。结论中药熏洗用于关节镜下关节清理术联合关节腔注射富血小板血浆治疗KOA中效果显著,不仅可调节TLR-4、IL-1β表达,还能促进膝关节功能恢复,值得推广。

Objective To explore the effect of traditional Chinese medicine fumigation and washing on arthroscopic joint debridement combined with intra-articular injection of platelet-rich plasma in the treatment of knee osteoarthritis(KOA)and its influences on the expression of toll-like receptor-4(TLR-4)and interleukin-1β(IL-1β).Methods A total of 50 patients with KOA admitted from January 2021 to July 2022 were selected as the research objects and divided into control group and observation group by random number table method,with 25 cases in each group.The control group was treated with arthroscopic joint debridement combined with intra-articular injection of platelet-rich plasma,and the observation group was treated with traditional Chinese medicine fumigation and washing on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of TLR-4 and IL-1βin the observation group were lower than those in the control group(P<0.05).After treatment,the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score of the observation group was lower than that of the control group,and the Lysholm knee score was higher than that of the control group(P<0.05).Conclusion The traditional Chinese medicine fumigation and washing for arthroscopic joint debridement combined with intra-articular injection of platelet-rich plasma in the treatment of KOA has a significant effect.It can not only regulate the expression of TLR-4 and IL-1β,but also promote the recovery of knee joint function,which is worthy of promotion.
