摘要
目的分析重症肺炎合并呼吸衰竭辅助应用支气管肺泡灌洗的方法及对临床疗效、炎性因子、肺功能的影响。方法选取江南大学附属医院重症医学科于2022年11月—2023年6月收治的80例重症肺炎合并呼吸衰竭患者作为研究对象,根据单双数法进行分组,双数纳入对照组(n=40)行常规治疗,单数纳入观察组(n=40)在对照组基础上行支气管肺泡灌洗辅助治疗。对比两组患者临床疗效及治疗前后炎性因子、肺功能指标变化情况。结果观察组的治疗有效率为90.00%,明显高于对照组的72.50%,差异有统计学意义(χ^(2)=4.021,P<0.05);观察组肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、C-反应蛋白(C-reactive Protein,CRP)水平均低于对照组,呼气峰流速(Peak Expiratory Flow,PEF)、第1秒用力呼气容积(Forced Expiratory Volume In One Second,FEV1)水平均优于对照组,差异有统计学意义(P均<0.05)。结论重症肺炎合并呼吸衰竭辅助应用支气管肺泡灌洗,可提高临床疗效,进一步抑制炎性反应。
Objective To analyze the methods of bronchoalveolar lavage assisted with severe pneumonia complicated with respiratory failure and its effects on clinical efficacy,inflammatory factors and lung function.Methods A total of 80 patients with severe pneumonia combined with respiratory failure admitted to the Department of Critical Care Medicine,Affiliated Hospital of Jiangnan University from November 2022 to June 2023 were selected as the study objects.The patients were divided into the observation group(n=40)and the control group(n=40)according to the odd-even number method.The control group received routine treatment,and the observation group received basal ascending bronchoalveolar lavage adjuvant treatment.The clinical efficacy and the changes of inflammatory factors and lung function indexes before and after treatment were compared between the two groups.Results The effective rate of the observation group was 90.00%,which was significantly higher than that of the control group(72.50%),and the difference was statistically significant(χ^(2)=4.021,P<0.05).The levels of tumor necrosis factor-α(TNF-α)and c-reactive protein(CRP)in the observation group were lower than those in the control group,and the levels of peak expiratory flow(PEF)and forced expiratory volume in one second(FEV1)were better than those of control group,and the differences were statistically significant(all P<0.05).Conclusion The adjuvant bronchoalveolar lavage with severe pneumonia combined with respiratory failure can enhance the clinical effect and further inhibit the inflammatory response.
