纤维支气管镜吸痰联合肺泡灌洗治疗老年坠积性肺炎患者的效果及对氧合指数的影响

Effect of Sputum Aspiration Combined with Alveolar Lavage in the Treatment of Elderly Patients with Hypostatic Pneumonia and Its Influence on Oxygenation Index

目的探讨纤维支气管镜吸痰联合肺泡灌洗治疗坠积性肺炎患者的效果。方法选取2020年1月—2022年12月沛县人民医院重症医学科收治的60例重症肺部感染患者为研究对象,根据随机数表法分成两组,各30例。对照组实施常规治疗方法,观察组在对照组基础上采用纤维支气管镜吸痰联合肺泡灌洗疗法,比较两组感染指标、动脉血气指标、肺部炎症吸收情况及不良反应发生情况。结果治疗后观察组患者血清C反应蛋白、降钙素原、白介素-6、二氧化碳分压低于对照组,差异有统计学意义(P均<0.05)。治疗后观察组患者的氧分压,氧合指数高于对照组,差异有统计学意义(P均<0.05);观察组的疗效为93.33%,优于对照组的73.33%,差异有统计学意义(χ^(2)=4.320,P<0.05)。治疗期间,两组未发生不良反应。结论纤维支气管镜吸痰联合肺泡灌洗治疗坠积性肺炎患者的辅助治疗,可改善动脉血气指标,降低炎症因子水平,并且不会增加不良反应的发生率,患者安全性和有效性能够得到保障。

Objective To investigate the effect of sputum aspiration combined with alveolar lavage on patients with hypostatic pneumonia.Methods A total of 60 patients with severe pulmonary infection admitted to the Department of Critical Care Medicine of Peixian County People's Hospital from January 2020 to December 2022 were selected as the study objects and divided into two groups with 30 cases each according to random number table method.The control group was treated with conventional treatment,and the observation group was treated with sputum aspiration combined with alveolar lavage based on the control group.Infection indicators,arterial blood gas index,pulmonary inflammatory absorption and adverse reactions occurrence were compared between the two groups.Results After treatment,serum C-reactive protein,procalcitonin,interleukin-6 and carbon dioxide in the observation group were lower than the control group,and the difference was statistically significant(all P<0.05).After treatment,the oxygen partial pressure and oxygenation index of the observation group were higher than that in the control group,with statistically significant differences(all P<0.05).The efficacy of the observation group was 93.33%,which was better than that of the control group(73.33%),and the difference was statistically significant(χ^(2)=4.320,P<0.05).No adverse reactions occurred during treatment between two groups.Conclusion Sputum aspiration combined with alveolar lavage in the treatment of patients with hypostatic pneumonia can improve arterial blood gas indexes,reduce the level of inflammatory factors,and do not increase the incidence of adverse reactions,and the safety and effectiveness of patients can be guaranteed.
