Mb、cTnⅠ、CK-MB、BNP联合检测对早期急性心肌梗死的诊断价值

摘要 目的探讨肌红蛋白(Myoglobin,Mb)、心肌肌钙蛋白Ⅰ(Cardiac Troponin,cTnⅠ)、肌酸激酶同工酶(Creatine Kinase Isoenzymes-MB,CK-MB)、脑利钠肽(Brain Natriuretic Peptide,BNP)联合检测对诊断早期急性心肌梗死的价值。方法选取2020年1月—2022年12月丹阳市人民医院收治的100例早期急性心肌梗死患者与100名健康体检者为研究对象,分别设为心梗组与健康组,均检测Mb、cTnⅠ、CK-MB、BNP水平,比较两组检测值的差异,另根据临床结果,评估联合检测对急性心肌梗死的诊断效能。结果心梗组Mb(92.84±22.52)ng/mL、cTnⅠ(4.96±0.78)ng/mL、CK-MB(48.91±14.62)U/L、BNP(1510.71±268.40)pg/mL,均高于健康组,差异有统计学意义(t=26.425、63.200、22.761、54.726,P均<0.05)。联合检测诊断准确率95.50%、灵敏度96.00%,高于单独检测值,差异有统计学意义(P均<0.05);联合检测诊断特异度95.00%,与单独检测对比,差异无统计学意义(P均>0.05)。结论早期急性梗死患者Mb、cTnⅠ、CK-MB、BNP水平明显异常,联合检测该4项指标可提高诊断效能。 Objective To investigate the value of combined detection of myoglobin(Mb),troponin(cTnI),creatine kinase isoenzyme(CK-MB)and brain natriuretic peptide(BNP)in the diagnosis of early acute myocardial infarction.Methods A total of 100 patients with early acute myocardial infarction admitted to the hospital from January 2020 to December 2022 and 100 healthy medical checkups were selected as the infarction group and the healthy group respectively,and the levels of Mb,cTnI,CK-MB,and BNP were detected in both groups to compare the differences in the values between the two groups,and the diagnostic efficacy of the combined test for acute myocardial infarction was evaluated according to the clinical results.Results Mb(92.84±22.52)ng/mL,cTnI(4.96±0.78)ng/mL,CK-MB(48.91±14.62)U/L,and BNP(1510.71±268.40)pg/mL in the infarction group were higher than those in the healthy group,and the differences were statistically significant(t=26.425,63.200,22.761,54.726,all P<0.05).The diagnostic accuracy of 95.50% and sensitivity of 96.00% of the combined test were higher than the value of the test alone,and the differences were statistically significant(all P<0.05).The specificity of the combined test was 95.00%,which was not significantly different from the test alone,and the differences were not statistically significant all(all P>0.05).Conclusion The levels of Mb,cTnI,CK-MB and BNP were obviously abnormal in patients with early acute infarction,and the combined detection of these four indexes can improve the diagnostic efficacy.
