氢质子磁共振波谱成像结合视频脑电图诊断癫痫的价值

Clinical value of hydrogen proton magnetic resonance spectroscopy imaging combined with video electroencephalography to diagnose epilepsy

摘要 目的探讨氢质子磁共振波谱成像结合视频脑电图对癫痫诊断的临床价值。方法选取2021-06—2023-06湖北医药学院附属东风总医院收治的65例疑似癫痫患者为研究对象,比较分析氢质子磁共振波谱成像、视频脑电图检查及联合检查的敏感性、特异性、准确性之间的差异,并制作3种检查方法的ROC曲线。结果65例疑似癫痫患者中,确诊阳性50例(76.92%),确诊阴性15例(23.08%)。3种检查方法特异度比较,差异无统计学意义(P>0.05)。联合检查、氢质子磁共振波谱成像及视频脑电图检查的灵敏度、准确率依次降低,差异有统计学意义(P<0.05)。氢质子磁共振波谱成像、视频脑电图检查、联合检查诊断脑梗死的ROC曲线下面积(AUC)分别为0.8187、0.7766、0.8519。结论氢质子磁共振波谱成像结合视频脑电图有利于提高癫痫患者诊断的灵敏度、准确率及AUC水平,值得推广应用。 Objective To investigate the clinical value of hydrogen proton magnetic resonance spectroscopy imaging combined with video electroencephatography(EEG)for epilepsy.Methods From June 2021 to June 2023,65 suspected patients with epilepsy admitted to Sinopharm Dongfeng General Hospital,Hubei University of Medicine were selected as the research objects,and the differences between the sensitivity,specificity and accuracy of hydrogen proton magnetic resonance spectroscopy,video electroencephalogram and combined examination were compared and analyzed,and the ROC curves of the three examination methods were made.Results Of the 65 patients with suspected epilepsy,50(76.92%)were positive and 15(23.08%)were negative.The specificity of the three test methods was not significant(P>0.05).The sensitivity and accuracy of combined examination,hydrogen proton MR spectroscopy imaging and video EEG were reduced successively reduced,and the difference was statistically significant(P<0.05).The AUC under the ROC curve of hydrogen proton magnetic resonance spectroscopy imaging,video EEG examination,and combined examination for the diagnosis of cerebral infarction was 0.8187,0.7766,and 0.8519,respectively.Conclusion Hydrogen proton magnetic resonance spectroscopy imaging combined with video EEG is beneficial to improve the diagnostic sensitivity,accuracy and AUC level of epilepsy patients,which is worth popularization and application.
