摘要
目的探究臀中肌渐进性抗阻训练对脑性瘫痪儿童站立平衡和步行能力的影响。方法选取2019年1月—2023年6月在重庆綦江爱康康复医院康复科住院的痉挛型偏瘫脑瘫(SHCP)患儿46例,随机分为试验组和对照组,每组23例。对照组患儿采用常规0.5 Kg沙袋恒定阻力训练,试验组患儿在常规训练过程中增加臀中肌渐进性抗阻训练(弹力带)。2组患儿每次训练30 min,每天1次,每周5 d,连续训练12周。比较2组患儿治疗前后的徒手肌力评定分级(MMT)、Berg平衡量表(BBS)及粗大运动功能测试量表(GMFM)评分。结果治疗前,2组患儿的MMT、BBS及GMFM评分比较,差异均无统计学意义(P>0.05);治疗后,2组患儿MMT、BBS及GMFM评分均高于本组治疗前,差异均有统计学意义(P<0.05)。结论臀中肌渐进性抗阻训练可以改善脑性瘫痪儿童站立平衡和步行能力。
Objective To explore the effect of progressive resistance training of gluteus medius on standing balance and walking ability in children with cerebral palsy.Methods Forty-six children with spastic hemiplegic cerebral palsy(SHCP)hospitalized in the Department of Rehabilitation Medicine of our Hospital from January 2019 to June 2023 were selected and randomly assigned to an experimental group and a control group,with 23 cases in each group.Children in the control group underwent conventional resistance training with a constant resistance of 0.5 kg sandbag,while those in the experimental group received progressive resistance training of gluteus medius(using elastic bands)in addition to conventional training.Participants in both groups received training for 30 minutes each time,once a day,5 days a week,for 12 consecutive weeks.The manual muscle testing(MMT)grade,Berg Balance Scale(BBS),and Gross Motor Function Measure(GMFM)scores before and after treatment were compared between the two groups.Results Before treatment,no significant difference was observed in MMT,BBS,and GMFM scores between the two groups(P>0.05);after treatment,the MMT,BBS,and GMFM scores of both groups were significantly higher than those before treatment(P<0.05).Conclusion Progressive resistance training of gluteus medius can promote standing balance and walking ability in children with cerebral palsy.
