Ⅰ-Ⅱa期宫颈癌患者术后盆腔小野放疗的临床效果

摘要 目的观察Ⅰ-Ⅱa期宫颈癌患者术后采用盆腔小野放疗的生存率及不良反应发生情况。方法选取我院2017年1月—2018年1月已接受广泛全子宫切除+盆腔区域淋巴结清扫术且具有肿瘤复发高中危因素的30例Ⅰ-Ⅱa期宫颈癌患者,进行盆腔小野放射治疗,并观察患者的生存率及不良反应发生情况。结果30例患者中,2年生存率、无疾病进展率分别为100.0%、96.7%,30例患者均具有良好的耐受性,无3级以上直肠、泌尿生殖道不良反应发生。结论对具有预后不良危险因素并已行根治性手术的Ⅰ-Ⅱa期宫颈癌患者,进行盆腔小野放疗,其结果与文献报道和我院既往同类患者放疗标准野照射进行比较,发现盆腔小野照射在保证疗效的前提下能够明显降低不良反应发生风险,提高患者的生活质量,在临床上是可行的。 Objective To observe the survival rate and occurrence of adverse reactions in patients with stageⅠ-Ⅱa cervical cancer who underwent postoperative pelvic field radiotherapy.Methods Thirty patients with stageⅠ-Ⅱa cervical cancer who underwent extensive radical hysterectomy plus pelvic lymph node dissection and had high to intermediate risk factors for tumor recurrence in our hospital from January 2017 to January 2018 were selected for this study.They received postoperative pelvic field radiotherapy,and their survival rates and occurrence of adverse reactions were observed.Results Among the 30 patients,the 2-year survival rate and disease-free progression rate were 100.0%and 96.7%,respectively.All 30 patients tolerated the treatment well,with no grade 3 or higher adverse reactions in the rectum,urinary or reproductive tracts.Conclusion For patients with stageⅠ-Ⅱa cervical cancer who have poor prognostic factors and have undergone radical surgery,pelvic field radiotherapy can significantly reduce the risk of adverse reactions and raise the quality of life while ensuring efficacy.Compared with standard field irradiation used in our hospital and reported in the literature for similar patients,pelvic field irradiation is clinically feasible.
