摘要
目的探讨高频超声对真皮纤维瘤的诊断价值及不同部位病灶的超声表现分析。方法回顾性分析2017年1月—2019年12月在我院进行体表肿物彩超检查,且经病理证实为真皮纤维瘤患者13例,使用GE LOGIQ E9彩色多普勒超声诊断仪,采用高频线阵探头9~15 MHz多切面扫查病灶,并记录病灶部位、大小、边界、内部及后方回声、内部及外周血流情况。结果总病灶个数为14个,单发者12例(92.31%),多发者1例(7.69%)。位于躯干7个(50.00%),四肢5个(35.71%),头颈部2个(14.29%)。病灶规则10个(71.43%),病灶不规则4个(28.57%)。病变长径平均(8.00±1.5)mm,短径平均(4.54±1.73)mm。病灶位于真皮层及其上部5个(35.71%),病灶累及真皮层并蔓延至皮下9个(64.29%);边界清晰11个(78.57%),边界欠清3个(21.43%)。内部回声均匀1个(7.14%),无回声2个(14.29%),内部回声不均匀11个(78.57%);后方回声稍增强5个(35.71%),其余为后方回声无明显变化9个(64.29%)。内部有血流信号2个(14.29%),内部无血流信号12个(85.71%)。结论高频超声对真皮纤维瘤的诊断有一定的价值,必要时结合病理诊断可提高临床确诊率。
Objective To explore the diagnostic value of high-frequency ultrasound and analyze the ultrasonic features of dermatofibroma in different locations.Methods Thirteen patients diagnosed with dermatofibroma by pathology who underwent surface mass color Doppler ultrasonography in our hospital from January 2017 to December 2019 were retrospectively analyzed.GE LOGIQ E9 color Doppler ultrasound diagnostic equipment was used with a high-frequency linear array probe(9-15 MHz)to scan the lesions in multiple sections,recording the location,size,border,internal and posterior echoes,and internal and peripheral blood flow of the lesions.Results There were a total of 14 lesions,with 12 cases(92.31%)being solitary and 1 case(7.69%)being multiple.They were located on the trunk in 7 cases(50.00%),limbs in 5 cases(35.71%),and head and neck in 2 cases(14.29%).Overall,10 lesions(71.43%)were regular,and 4 lesions(28.57%)were irregular.The mean long diameter of the lesions was(8.00±1.5)mm,and the mean short diameter was(4.54±1.73)mm.A total of 5 lesions(35.71%)were located in the dermis and its upper part,and 9 lesions(64.29%)involved the dermis and extended into the subcutaneous tissue.Eleven lesions(78.57%)had clear borders,and 3 lesions(21.43%)had unclear borders.One lesion(7.14%)had homogeneous internal echoes,2 lesions(14.29%)had no echoes,and 11 lesions(78.57%)had heterogeneous internal echoes.Five lesions(35.71%)showed slightly enhanced posterior echoes,while the rest(64.29%)showed no marked change in posterior echoes.Two lesions(14.29%)showed blood flow signals internally,while 12 lesions(85.71%)showed no blood flow signals.Conclusion High-frequency ultrasound has a certain value in the diagnosis of dermatofibroma,and when necessary,combined with pathological diagnosis,it can potentiate the clinical diagnostic rate.
