不同剂量叶酸联合维生素B_(12)治疗老年高血压合并高同型半胱氨酸血症的临床效果

Clinical Effect of Different Doses of Folic Acid Combined with Vitamin B_(12) in the Treatment of Senile Hypertension Complicated with Hyperhomocysteinemia

目的:分析不同剂量叶酸联合维生素B_(12)治疗老年高血压合并高同型半胱氨酸血症(HHcy)的应用效果.方法:选取2023年1—12月北京市延庆区妇幼保健院就诊的老年高血压合并HHcy患者67例作为研究对象,以随机数字表法分为观察1组(33例,小剂量叶酸联合维生素B_(12)治疗)和观察2组(34例,大剂量叶酸联合维生素B_(12)治疗).比较两组同型半胱氨酸(Hcy)及叶酸水平、收缩压(SBP)及舒张压(DBP)水平、不良反应发生率.结果:治疗前,两组患者Hcy及叶酸水平比较,差异无统计学意义(P>0.05);治疗后,两组患者Hcy水平均低于治疗前,叶酸水平均高于治疗前,观察2组优于观察1组,差异有统计学意义(P<0.05).治疗前,两组患者SBP及DBP水平比较,差异无统计学意义(P>0.05);治疗后,两组患者SBP及DBP水平均低于治疗前,观察2组低于观察1组,差异有统计学意义(P<0.05).两组均未出现过敏、肝损伤、肾损伤等不良反应.结论:叶酸联合维生素B_(12)在老年高血压合并HHcy治疗中的应用效果显著,其中采用0.8 mg叶酸的效果较佳,可有效改善Hcy及叶酸水平,稳定血压,且不良反应未增加,具有临床推广价值.

Objective:To analyze the effect of different doses of folic acid combined with vitamin B_(12) in the treatment of elderly hypertension complicated with hyperhomocysteinemia(HHcy).Methods:A total of 67 elderly patients with hypertension combined with HHcy treated in Beijing Yanqing District Maternal and Child Health Hospital from January to December 2023 were selected as study subjects,and divided into observation group 1(33 cases,low-dose folic acid combined with vitamin B_(12) treatment)and observation group 2(34 cases,high-dose folic acid combined with vitamin B_(12) treatment)by random number table method.The levels of homocysteine(Hcy)and folate,systolic blood pressure(SBP)and diastolic blood pressure(DBP),and the incidence of adverse reactions were compared between the two groups.Results:Before treatment,there was no significant difference in Hcy and folic acid levels between two groups(P>0.05);after treatment,the Hcy level in both groups was lower than before treatment,and folic acid level was higher than before treatment,observation group 2 was better than observation group 1,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in SBP and DBP levels between two groups(P>0.05);after treatment,SBP and DBP levels in two groups were lower than before treatment,and observation group 2 was lower than observation group 1,the difference was statistically significant(P<0.05).There were no adverse reactions such as allergy,liver injury and kidney injury in both groups.Conclusion:The application effect of folic acid combined with vitamin B_(12) in the treatment of elderly hypertension combined with HHcy is significant,and the effect of 0.8 mg folic acid is better,which can effectively improve the levels of Hcy and folic acid and stabilize blood pressure without increasing adverse reactions,and has clinical promotion value.
