针刺联合药棒穴位按摩治疗腰椎间盘突出症的效果及对疼痛程度、腰椎功能的影响

Effect of Acupuncture Combined with Acupuncture Stick Acupoint Massage on Lumbar Disc Herniation and Its Influ- ence on Pain Degree and Lumbar Function

目的:探讨针刺联合药棒穴位按摩治疗腰椎间盘突出症的效果及对疼痛程度、腰椎功能的影响.方法:选取2020年6月—2023年6月北京积水潭医院贵州医院收治的腰椎间盘突出症患者100例作为研究对象,应用随机数字表法分为研究组和对照组,各50例.对照组给予常规针刺治疗,研究组在对照组基础上给予药棒穴位按摩治疗.比较两组治疗总有效率、中医证候积分、腰椎功能评分、疼痛评分.结果:研究组治疗总有效率高于对照组,差异有统计学意义(P=0.004);治疗后,两组血瘀证、寒湿证、湿热证、肝肾亏虚证积分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);治疗后,两组Oswestry功能障碍指数评分均低于治疗前,日本骨科协会评估治疗评分均高于治疗前,且研究组优于对照组,差异有统计学意义(P<0.05);治疗后,两组数字模拟评分法评分均低于治疗前,研究组低于对照组,差异有统计学意义(P<0.05).结论:针刺联合药棒穴位按摩治疗腰椎间盘突出症的效果显著,可有效减轻患者疼痛,改善腰椎功能及临床症状.

Objective:To investigate the effect of acupuncture combined with acupuncture stick acupoint massage on lumbar disc herniation and its influence on pain degree and lumbar function.Methods:A total of one hundred patients with lumbar disc herniation admitted to Beijing Jishuitan Hospital Guizhou Hospital from June 2020 to June 2023 were selected as the study objects and divided into study group and control group by random number table method,with fifty cases in each group.The control group was given conventional acupuncture treatment,and the study group was given acupoint massage based on the control group.The total effective rate,TCM syndrome score,lumbar function score and pain score were compared between the two groups.Results:The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P=0.004);after treatment,the scores of blood stasis syndrome,cold dampness syndrome,damp-heat syndrome and liver and kidney deficiency syndrome in two groups were lower than before treatment,and the study group was lower than the control group,the difference was statistically significant(P<0.05);after treatment,the scores of Oswestry disability index in both groups were lower than before treatment,and the scores evaluated by the Japanese orthopaedic association were higher than before treatment,and the study group was better than the control group,with statistical significance(P<0.05);after treatment,the scores of the two groups were lower than before treatment,and the study group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion:Acupuncture combined with drug stick acupoint massage can effectively reduce pain and improve lumbar function and clinical symptoms of lumbar disc herniation.
