基于直方图的CT检查技术在甲状腺结节性质鉴别中的作用研究

Role of Histogram Based CT Technique in the Identification of Thyroid Nodules

目的:分析CT图像直方图在鉴别甲状腺结节性质中的作用.方法:选取2022年8月—2023年8月上海市奉贤区精神卫生中心收治的甲状腺结节患者80例(86个结节)作为研究对象,根据结节良恶性(手术病理结果)分为恶性组和良性组,各40例.患者均开展CT平扫与增强扫描.围绕轴位病灶,借助Mazda软件勾画感兴趣(ROI),开展直方图分析,比较结节良、恶性的差异情况,利用受试者工作特征(ROC)曲线评定直方图参数在鉴别诊断甲状腺性质中的效能.结果:在CT平扫中,恶性结节的均值及第10、50、90百分位数高于良性结节,差异有统计学意义(P<0.05);在CT增强扫描中,恶性结节的均值及第1、10、50、90百分位数高于良性结节,方差低于良性结节,差异有统计学意义(P<0.05).ROC曲线结果显示,CT平扫直方图参数中曲线下面积(AUC)最高的为均值(0.662);在增强扫描直方图参数中,AUC值最高的是第10百分位数(0.683).结论:CT直方图分析在鉴别甲状腺结节性质中具有一定价值,可将其作为一种辅助手段.

Objective:To analyze the role of CT image histogram in distinguishing thyroid nodule properties.Methods:Eighty patients with thyroid nodules(86 nodules)admitted to Shanghai Fengxian District Mental Health Center from August 2022 to August 2023 were selected as the study objects.According to benign and malignant nodules(surgical pathological results),they were divided into malignant group and benign group,with forty cases each.All patients underwent CT plain scan and enhanced scan.Around the axial lesions,Mazda software was used to sketch ROI,and histogram analysis was carried out to compare the difference between benign and malignant nodules.Receiver operating characteristic(ROC)curve was used to evaluate the efficacy of histogram parameters in differential diagnosis of thyroid properties.Results:In plain CT scan,the mean value and 10th,50th and 90th percentiles of malignant nodules were higher than those of benign nodules,with statistical significance(P<0.05);in enhanced CT scan,the mean value and the 1st,10th,50th and 90th percentiles of malignant nodules were higher than those of benign nodules,and the variance was lower than that of benign nodules,with statistical significance(P<0.05).ROC curve results showed that the area under the curve(AUC)of CT plain scan histogram was the highest(0.662);among the enhanced scan histogram parameters,the highest AUC value is the 10th percentile(0.683).Conclusion:CT histogram analysis is valuable in identifying thyroid nodule properties and can be used as an auxiliary means.
