兰州地区乙型及丙型病毒性肝炎基因型分布研究

Study on genotype distribution of hepatitis B and C in Lanzhou

目的探讨兰州地区乙型肝炎病毒(HBV)与丙型肝炎病毒(HCV)基因型分布特点。方法采用PCR-荧光探针法对2018年1月至2023年8月在该院就诊的1130例HBV感染者、1781例HCV感染者进行基因型检测。采用SPSS26.0统计学软件进行数据分析。结果HBV感染者基因型分布以C型最多,为925例(82.0%),其次为B型124例(11.0%);B、C、D基因型间患者性别分布比较,差异无统计学意义(χ^(2)=3.66,P=0.09),而在各年龄段患者中比较,差异有统计学意义(χ^(2)=33.88,P=0.01)。HCV感染者基因型分布主要为2a型930例(52.2%),其次为1b型737例(41.4%);各基因型间患者性别分布比较,差异有统计学意义(χ^(2)=46.31,P=0.01);各年龄段患者1b、2a基因型分布比较,差异有统计学意义(χ^(2)=18.36,P=0.01)。结论在兰州地区病毒性肝炎患者中,HBV C型与HCV 2a型为主要感染基因型;HBV B、C、D型分布与年龄有关;HCV各基因型分布与性别有关,HCV 1b、2a型分布因年龄不同而有差异。

Objective To investigate the genotype distribution characteristics of hepatitis B virus(HBV)and hepatitis C virus(HCV)in Lanzhou.Methods From January 2018 to August 2023,1130 patients with HBV infection and 1781 patients with HCV infection in the hospital were genotyped by PCR-fluorescent probe method.SPSS26.0 statistical software was used to analyze the data.Results Genotype C was the most common genotype(925 cases,82.0%),followed by genotype B(124 cases,11.0%).There was no significant difference in gender distribution among genotypes B,C and D(χ^(2)=3.66,P=0.09),but there was significant difference among different age groups(χ^(2)=33.88,P=0.01).Genotype 2a was the most common genotype(930 cases,52.2%),followed by genotype 1b(737 cases,41.4%).There was significant difference in gender distribution among different genotypes(χ^(2)=46.31,P=0.01).There were significant differences in the distribution of genotype 1b and 2a among different age groups(χ^(2)=18.36,P=0.01).Conclusion HBV C genotype and HCV 2a genotype are the main infection genotypes in patients with viral hepatitis in Lanzhou.The distribution of HBV genotypes B,C and D was related to age.The distribution of HCV genotypes was related to gender,and the distribution of HCV 1b and 2a genotypes was different with age.
