经阴道结合经腹部彩色多普勒超声用于异位妊娠诊断的效能分析

Efficacy analysis of transvaginal ultrasound combined with transabdominal ultrasound for the diagnosis of ectopic pregnancy

目的:研究经阴道超声结合经腹部超声用于异位妊娠诊断的效能。方法:选取2021年3月—2023年3月青岛市城阳区人民医院收治的80例疑似异位妊娠患者为研究对象,所有患者均进行经阴道彩色多普勒(A检查)、经腹部彩色多普勒超声(B检查)以及腹腔镜或手术病理检查,并以腹腔镜或手术病理检查结果作为金标准,分析A、B单独检查与二者联合检查诊断异位妊娠的灵敏度、特异度、准确率以及对附件包块、胚芽、盆腔积液、心管搏动、卵黄囊等检出率。结果:A、B联合检查异位妊娠的灵敏度、准确率高于A、B单项独查,且A、B联合检查异位妊娠的特异度高于B单项独查,差异有统计学意义;A、B联合检查对异位妊娠心管搏动、卵黄囊的检出率高于B检查,且A、B联合检查对异位妊娠附件包块、胚芽及盆腔积液的检出率高于A、B检查检查,差异有统计学意义(P<0.05)。结论:相比于经腹部与经阴道彩色多普勒超声单项检查,二者联合检查可通过不同影像特征提高异位妊娠的检出率,诊断异位妊娠的效能较高。

Objective To study the diagnostic efficiency related indexes of ectopic pregnancy by different examination methods.Methods This was a retrospective study,and 80 subjects(suspected ectopic pregnancy patients)were included from March 2021 to March 2023.All patients underwent transvaginal(A)and transabdominal color Doppler ultrasound(B)and laparoscopic or surgical pathological examination,and the results of laparoscopic or surgical pathology were taken as the gold standard.A unified analysis of all indicators were conducted.Results Compared with the data of single examination,the data related to the diagnostic efficiency of the combination of the two examinations(A and B examination)were better,and the result of statistical processing was P<0.05.Compared with the data of single examination(the detection rate of adnexal masses,germ and pelvic effusion),the detection rate of 59 patients with ectopic pregnancy by combined examination was better.However,the detection rate of cardiac pulsation and yolk sac was only higher than that of B examination,and the result of statistical treatment was P<0.05.Conclusion Compared with single examination(transabdominal and transvaginal color Doppler ultrasound),the overall diagnostic efficiency of the combination is higher,and the detection rate of different characteristics is higher,which has certain reference value in the early diagnosis of the disease.
