前馈控制理念联合核心稳定训练在腰椎管狭窄症中的应用

摘要 目的:探讨前馈控制理念联合核心稳定性训练干预在腰椎管狭窄症(LSS)中的应用。方法:选取2021年6月—2022年6月收治的LSS行腰椎管手术的患者89例,采用随机数字表法分为观察组(45例)和对照组(44例)。对照组采用核心稳定性训练干预,观察组采用前馈控制理念联合核心稳定性训练干预,两组均干预8周。比较两组患者腰背肌群功能、腰椎功能、自我效能感以及生活质量。结果:干预8周后,观察组峰力矩(PT)、平均功率(AP)水平显著高于对照组,差异有统计学意义(P<0.05);干预8周后,观察组腰背屈伸比(F/E)水平显著低于对照组,差异有统计学意义(P<0.05);干预8周后,观察组患者Oswestry功能障碍指数(ODI)评分显著低于对照组,差异有统计学意义(P<0.05);干预8周后,观察组患者日本骨科协会评估治疗分数(JOA)、自我效能感量表(GSES)评分显著高于对照组,差异有统计学意义(P<0.05);干预8周后,观察组生活质量评分显著高于对照组,差异有统计学意义(P<0.05)。结论:前馈控制理念联合核心稳定性训练能有效改善患者腰背肌群障碍,促进腰背功能康复,提高自我效能感,改善生活质量。 Objective:To explore the application of feedforward control concept combined with core stability training intervention in lumbar spinal stenosis(LSS).Methods:All 89 patients with LSS who underwent lumbar spinal surgery from June 2021 to June 2022 were selected and randomly divided into observation group(45 cases) and control group(44 cases) using a random number table method.The control group patients received core stability training intervention,while the observation group patients received feedforward control concept combined with core stability training intervention.Both groups of patients received 8 weeks of intervention.The function of the lower back muscle group,lumbar spine function,self-efficacy,and quality of life between two groups of patients were compared.Results:After 8weeks of intervention,the peak torque(PT) and average power(AP) levels in the observation group were significantly higher than those in the control group(P<0.05).After 8 weeks of intervention,the level of waist back flexion extension ratio(F/E) in the observation group was significantly lower than that in the control group(P<0.05).After 8weeks of intervention,Oswestry dysfunction index(ODI) score of the observation group patients was significantly lower than that of the control group patients(P<0.05).After 8 weeks of intervention,Japanese Orthopedic Association assessment treatment score(JOA) and self-efficacy scale(GSES) scores of the observation group patients were significantly higher than those of the control group patients(P<0.05).After 8 weeks of intervention,the quality of life score of the observation group patients was significantly higher than that of the control group patients(P<0.05).Conclusion:The combination of feedforward control concept and core stability training for postoperative intervention in LSS patients can effectively improve the disorder of the lower back muscle group,promote the recovery of lower back function,improve self-efficacy.
