血清肿瘤坏死因子受体相关蛋白1、三叶因子1表达水平对卵巢癌患者化疗疗效的预测价值

Value of serum TRAP1 and TFF1 expression levels in predicting chemotherapy efficacy in ovarian cancer patients

摘要 目的 探讨化疗前血清指标肿瘤坏死因子受体相关蛋白1(TRAP1)、三叶因子1(TFF1)表达水平在预测卵巢癌患者化疗疗效中的临床价值。方法 回顾性分析2022年2月—2023年5月本院收治的85例卵巢癌患者的临床资料,根据化疗疗效分为无效组(30例)和有效组(55例)两组。比较两组患者临床特征及血清指标TRAP1、TFF1表达水平;建立受试者工作特征(ROC)曲线分析化疗前血清TRAP1、TFF1表达水平对卵巢癌患者化疗疗效的联合预测价值,计算曲线下面积(AUC)并进行Z检验比较;采用多因素logistic回归分析进行卵巢癌患者化疗疗效的影响因素分析。结果 无效组化疗前、化疗后血清TRAP1和TFF1表达水平均高于有效组(P<0.05),有效组化疗前血清TRAP1和TFF1表达水平高于化疗后(P<0.05)。无效组卵巢癌患者的肿瘤直径≥5 cm比例和FIGO分期Ⅳ期比例高于有效组(P<0.05)。血清TRAP1、TFF1表达水平单独及联合预测卵巢癌患者化疗疗效的AUC分别为0.851(95%CI:0.784~0.918)、0.860(95%CI:0.790~0.920)、0.938(95%CI:0.883~0.984),敏感度分别为90.98%、66.76%、86.78%,特异度分别为69.11%、92.74%、81.89%。TRAP1、TFF1、FIGO分期是卵巢癌患者化疗无效的独立危险因素(P<0.05)。结论 血清TRAP1和TFF1表达水平联合检测在预测卵巢癌患者化疗疗效中有较高的应用价值,较高的TRAP1和TFF1水平提示卵巢癌患者化疗无效。 Objective To explore the clinical value of serum indicators such as tumor necrosis factor receptor associated protein 1(TRAP1)and trefoil factor 1(TFF1)expression levels before chemotherapy in predicting chemotherapy efficacy in patients suffering from ovarian cancer.Methods A retrospective analysis was conducted on the clinical data of 85 patients suffering from ovarian cancer who visited our hospital during February 2022 and May 2023.According to the efficacy of chemotherapy,they were grouped into the ineffective group(30 cases)and the effective group(55 cases).The clinical characteristics and serum indicators TRAP1 and TFF1 expression levels were compared between two groups.Receiver operating characteristic(ROC)curve was established to analyze the combined predictive value of serum TRAP1 and TFF1 expression levels in ovarian cancer patients before chemotherapy.The area under the curve(AUC)was calculated and Z-test was performed for comparison.Multivariate logistic regression analysis was applied to analyze the influencing factors of chemotherapy efficacy among patients suffering from ovarian cancer.Results The expression levels of serum TRAP1 and TFF1 in the ineffective group were obviously higher than those in the effective group both at before and after chemotherapy(P<0.05).Expression levels of serum TRAP1 and TFF1 in the effective group before chemotherapy were obviously higher than those after chemotherapy(P<0.05).The proportion of tumor diameter over or equal to 5 cm and the proportion of FIGO stage IV in the ineffective group were obviously higher than those in the effective group(P<0.05).The AUC for predicting chemotherapy efficacy in patients suffering from ovarian cancer by using serum TRAP1 and TFF1 expression levels alone and in combination were 0.851(95%CI:0.784~0.918),0.860(95%CI:0.790~0.920),0.938(95%CI:0.883~0.984);the sensitivity were 90.98%,66.76%,and 86.78%,respectively;and specificity were 69.11%,92.74%,and 81.89%,respectively.TRAP1,TFF1,and FIGO staging were independent risk factors for ineffective chemotherapy in patients suffering from ovarian cancer(P<0.05).Conclusions The combined detection of serum TRAP1 and TFF1 expression levels has high application value in predicting chemotherapy efficacy in patients suffering from ovarian cancer.Highlevels of TRAP1 and TFF1 suggest the ineffectiveness of chemotherapy in ovarian cancer patients.
