摘要
目的 通过Meta分析评估超声双重造影(Double contrast-enhanced ultrasonography, DCEUS)特征对胃间质瘤(Gastric stromal tumors, GSTs)危险度分级的准确性。方法 在PubMed、Cochrane、Web of Science、Embase、 OVID-Medline、中国知网、万方、维普电子数据库中对符合纳入标准的相关研究进行了系统检索,并采用Revman 5.4及STATA 18.0软件进行Meta分析。结果 纳入的文献中纳入的研究质量均良好,4项研究(n=168)的Meta分析结果显示,敏感度为0.91(95%CI 0.69~0.98),特异度为0.84(95%CI 0.74~0.91),阳性似然比5.83(95%CI 3.51~9.67),阴性似然比0.11(95%CI 0.03~0.40),以及诊断比值比(DOR)52.91(95%CI 12.99~215.47),绘制合并受试者工作特征曲线(SROC),计算曲线下面积(AUC)为0.90(95%CI 0.87~0.92)。结论 DCEUS对胃间质瘤的诊断具有较高的敏感度和特异度,能够有效鉴别低危型和高危型胃间质瘤。
Objective To assess the accuracy of double contrast-enhanced ultrasonography(DCEUS)features for risk classification of gastric stromal tumors(GSTs)by Meta-analysis.Methods Related research which met the inclusion criteria were searched in in PubMed,Cochrane,Web of Science,Embase,OVID-Medline,CNKI,Wanfang,and VIP electronic databases,and Meta-analysis was performed by using Revman 5.4 and STATA 18.0 software.Results The quality of the included studies in the included literature was favorable,and the Meta-analysis results of the four studies(n=168)showed a sensitivity of 0.91(95%CI 0.69~0.98),a specificity of 0.84(95%CI 0.74~0.91),a positive likelihood ratio of 5.83(95%CI 3.51~9.67),and a negative likelihood ratio of 0.11(95%CI 0.03~0.40),and diagnostic odds ratio(DOR)52.91(95%CI 12.99~215.47),the combined subjects'work characterization curve(SROC)was plotted,and the area under the curve(AUC)was calculated as 0.90(95%CI 0.87~0.92).Conclusions DCEUS has high sensitivity and specificity for the diagnosis of gastric stromal tumors,and can effectively identify low-risk and high-risk types of gastric stromal tumors.
