巴里坤县城镇与牧区人群血浆致动脉粥样硬化指数与尿酸水平的相关性分析

摘要 目的分析巴里坤县城镇与牧区人群血浆致动脉粥样硬化指数(AIP)与尿酸水平的相关性。方法选取2018年9月至2021年3月在新疆维吾尔自治区哈密市巴里坤县医院进行体检的1388例体检人群(城镇人群675例,牧区人群713例)作为研究对象。按照是否患有高尿酸血症分别将城镇人群和牧区人群分为城镇高尿酸血症组(285例)、城镇对照组(390例)和牧区高尿酸血症组(141例)、牧区对照组(572例)。比较城镇高尿酸血症组和城镇对照组临床资料,以及牧区高尿酸血症组和牧区对照组临床资料。采用Spearman相关分析城镇高尿酸血症组和牧区高尿酸血症组的AIP与尿酸水平的相关性。采用多因素Logistic回归分析城镇人群及牧区人群发生高尿酸血症的危险因素。结果城镇高尿酸血症组体质量指数(BMI)、舒张压、AIP、尿酸、总胆固醇(TC)、甘油三酯(TG)水平高于城镇对照组,腰围大于城镇对照组,差异均有统计学意义(P<0.05)。牧区高尿酸血症组BMI、AIP、尿酸、TG水平高于牧区对照组,年龄、腰围大于牧区对照组,TC、高密度脂蛋白胆固醇(HDL-C)水平低于牧区对照组,差异均有统计学意义(P<0.05)。Spearman相关分析结果显示,城镇高尿酸血症组和牧区高尿酸血症组的AIP与尿酸水平均呈正相关(r_(s)=0.384、0.405,P<0.05)。多因素Logistic回归分析结果显示,BMI升高、腰围增大是城镇人群发生高尿酸血症的独立危险因素(P<0.05)。AIP升高,HDL-C、TC水平降低,腰围、年龄增大是牧区人群发生高尿酸血症的独立危险因素(P<0.05)。结论城镇与牧区人群AIP与尿酸水平密切相关,对预测该地区人群高尿酸血症的发生具有重要意义。 Objective To analyze the correlation between plasma atherogenic index(AIP)and uric acid level in urban and pastoral areas of Balikun County.Methods A total of 1388 people(675 cases in urban area and 713 cases in pastoral area)who underwent physical examination in Balikun County Hospital of Hami City,Xinjiang Uygur Autonomous Region from September 2018 to March 2021 were selected as the research subjects.According to the presence or absence of hyperuricemia,the urban and pastoral populations were divided into the urban hyperuricemia group(285 cases)and the urban control group(390 cases)and the pastoral hyperuricemia group(141 cases)and the pastoral control group(572 cases).The clinical data of the urban hyperuricemia group and the urban control group were compared,and the clinical data of the pastoral area hyperuricemia group and the pastoral area control group were compared.Spearman correlation was used to analyze the correlation between AIP and uric acid level in urban hyperuricemia group and pastoral hyperuricemia group.Multivariate Logistic regression was used to analyze the risk factors of hyperuricemia in urban area population and pastoral area population.Results The levels of body mass index(BMI),diastolic blood pressure,AIP,uric acid,total cholesterol(TC)and triglyceride(TG)in the urban hyperuricemia group were higher than those in the urban control group,and the waist circumference in the urban hyperuricemia group was greater than that in the urban control group,and the differences were statistically significant(P<0.05).The BMI,AIP,uric acid and TG levels in the pastoral hyperuricemia group were higher than those in the pastoral control group,and the age and waist circumference in the pastoral control group were greater than those in the pastoral control group,and the TC and high-density lipoprotein cholesterol(HDL-C)levels in the pastoral hyperuricemia group were lower than those in the pastoral control group,and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that AIP was positively correlated with uric acid level in both urban hyperuricemia group and pastoral hyperuricemia group(r_(s)=0.384,0.405,P<0.05).Multivariate Logistic regression analysis showed that the increase of BMI and waist circumference were independent risk factors for hyperuricemia in urban population(P<0.05).The increase of AIP,waist circumference and age and the decrease of TC and HDL-C were independent risk factors for hyperuricemia in pastoral population(P<0.05).Conclusion There is a close correlation between AIP and uric acid level in urban and pastoral populations,which has important significance in predicting the occurrence of hyperuricemia in urban and pastoral areas.
