Al^(18)F-NOTA-FAPI-04PET/CT在乳腺癌中的初步应用研究

摘要 目的探讨Al^(18)F-NOTA-FAPI-04PET/CT显像在乳腺癌诊断中的应用价值,并分析乳腺癌原发病灶Al^(18)F-NO-TA-FAPI-04摄取与其病理特征的关系。方法回顾分析2022-06-17-2022-11-09在中国医学科学院北京协和医学院北京协和医院行Al^(18)F-NOTA-FAPI-04PET/CT检查的29例初诊怀疑乳腺癌患者的临床资料。所有患者均经手术切除或穿刺活检确诊乳腺癌。在PET/CT图像上勾画各脏器及乳腺癌病灶的感兴趣区(ROI),获取各脏器及乳腺癌病灶的最大标准摄取值(SUV_(max))和平均标准摄取值(SUV_(mean))。比较不同病理特征乳腺癌PET/CT指标差异,符合正态分布的数据以X±S表示,2组间比较采用两独立样本t检验;不符合正态分布的数据用M(P25,P75)表示,2组间比较采用Mann-Whitney U检验,多组间比较采用Kruskal-Wallis H检验。结果体内显像示Al^(18)F-NOTA-FAPI-04经泌尿系统和肝胆系统排泄,其生理性摄取主要在涎腺、甲状腺、胰腺、肾皮质、子宫和肌肉。共发现31个乳腺癌原发病灶和28个淋巴结转移病灶,原发病灶SUV_(max)和SUV_(mean)分别为13.54±5.92和8.34±3.74,淋巴结转移病灶分别为8.94(4.86,15.42)和5.37(3.03,9.56)。不同病理特征乳腺癌原发病灶SUV_(max)和SUV_(mean)差异无统计学意义,均P>0.05。结论Al^(18)F-NOTA-FAPI-04高度浓聚于乳腺癌病灶,PET/CT图像对比度高,在乳腺癌的诊断中具有较好的临床应用价值。Al^(18)F-NOTA-FAPI-04摄取与乳腺癌病理特征无关。 Objective To investigate the value of Al^(18)F-NOTA-FAPI-04PET/CT imaging for clinical application in breast cancer,and to explore the correlation between the uptake of Al^(18)F-NOTA-FAPI-04in primary breast cancer and its pathological features.Methods From June 2022to November 2022,29patients who underwent Al^(18)F-NOTA-FAPI-04PET/CT with a primary diagnosis of suspected breast cancer were retrospectively analyzed.All patients were diagnosed with breast cancer by surgical resection or biopsy.The ROI of each organ and breast cancer lesion was outlined on PET/CT images,and the SUV_(max)and SUV_(mean)were obtained.The indices of different pathological factors of breast cancer were compared by the independent sample t test,Mann-Whitney Utest,and Kruskal Wallis Htest.Results In vivo imaging showed that Al^(18)F-NOTA-FAPI-04was excreted through urinary system and hepatobiliary system,and its physiological uptake was mainly in salivary gland,thyroid,pancreas,renal cortex,uterus and muscle.A total of 31breast cancer primary lesions and 28lymph node metastases were found.The SUV_(max)and SUV_(mean)of primary lesions were 13.54±5.92and 8.34±3.74,respectively.The SUV_(max)and SUV_(mean)of lymph node metastases were 8.94(4.86,15.42)and 5.37(3.03,9.56),respectively.There was no significant difference in SUV_(max)and SUV_(mean)of breast cancer primary lesions with different pathological features,all P>0.05.Conclusions Al^(18)F-NOTA-FAPI-04is highly concentrated in breast cancer lesions.Al^(18)F-NOTA-FAPI-04PET/CT imaging has high contrast and is clinically useful in the diagnosis of breast cancer.Al^(18)F-NOTA-FAPI-04uptake is not associated with pathological features of breast cancer.
