压缩感知技术用于评估膝关节半月板损伤

摘要 目的:探讨压缩感知(CS)技术用于半月板成像的可行性及对半月板损伤诊断和评估的价值。方法:31名初诊为膝关节外伤患者(外伤组)和15名无膝关节外伤史的健康志愿者(对照组)分别行膝关节MRI检查,采集CS-3D-MRI(压缩因子为9)序列图像,对照组采集质子密度加权频率衰减反转恢复序列(PDW-SPAIR)图像作为对比,两个序列扫描时间分别为2min49s和6min,并记录外伤组患者关节镜手术的检查结果。外伤组患者在关节镜手术前行MRI检查。以CS-DS_(strong)方式生成CS-3D-MRI序列图像,其重建CS图像的诊断结果与关节镜手术检查结果行一致性分析。采用表观信噪比(SNR)、对比信噪比(CNR)及图像质量评分评价对照组图像质量。结果:对照组重建CS图像和PDW-SPAIR图像信噪比分别为2.71±0.71和2.43±0.68,对比噪声比分别为-19.96±4.47和-17.39±4.53;图像质量评分分别为2.90±0.28和2.93±0.18,差异无统计学意义(P>0.05)。外伤组重建CS图像均显示半月板损伤,与关节镜手术检查结果一致性较强(Kappa=0.67,P<0.05)。结论:CS-3D-MRI可用于膝关节半月板成像,图像信噪比与常规PDW-SPAIR图像相当,并且能显著缩短成像时间,可应用于评估膝关节半月板损伤。 Objective:To explore the feasibility of compressed sensing(CS)for meniscus imaging and the value of diagnosis and evaluation for meniscus injury.Methods:MRI examination of knee joint was performed in 31 patients with initially knee injury(injury group)and 15 healthy volunteers without knee injury(control group),whose CS-3D-MRI(compression ratio of 9)were collected,while proton density weighted spectral attenuated inversion recovery(PDW-SPAIR)images were collected from the control group for comparison,the scanning time for these two sequences were 6 min and 2 min 49s,respectively,the results of arthroscopic examination in the injury group were recorded.Patients in the injury group received MRI examination before the arthroscopy,and the CS-3D-MRI were generated using the reconstruction of CS-DSstrong,and the consistency between the diagnosis results of reconstructed CS images and those of arthroscopic was analyzed.The image quality of control group was evaluated by apparent signal-to-noise ratio(SNR),contrast-to-noise ratio(CNR),and image quality score.Results:The SNR,CNR and image quality scores of reconstructed CS images and PDW-SPAIR images were 2.71±0.71 and 2.43±0.68,-19.96±4.47 and-17.39±4.53,2.90±0.28 and 2.93±0.18,respectively.All the reconstructed CS images showed meniscus injuries in the injury group,which had substantial consistency with the arthroscopic diagnosis results(Kappa=0.67,P<0.05).Conclusion:CS-3D-MRI can be used for knee meniscus imaging,the image signal-to-noise ratio is similar to that of the conventional PDW-SPAIR image,and the imaging time can be significantly shortened,which can be used to evaluate knee meniscus injury.
