腹腔镜虚拟模拟培训系统在腹腔镜手术缝合技能训练中的应用

Application of Laparoscopic Virtual Simulation Training System in Laparoscopic Surgical Suture Skills Training

目的探讨在普外科腹腔镜手术学员缝合技能训练中,通过虚拟模拟培训系统后取得的成效。方法选取2022年12月至2023年12月在伊犁哈萨克自治州友谊医院普外科学习的进修医师和实习生共60名,根据不同培训方式分为研究组(腹腔镜虚拟模拟培训系统)、对照组(手术缝合技能训练),每组30名,比较两组缝合熟练程度,吻合质量、操作失误发生率。结果通过缝合技能培训后,研究组打结质量、缝合协调度、调针和进针的评分均高于对照组(P<0.05);培训后研究组切缘外翻、吻合严密、针距匀称和左右对称的评分均高于对照组,统计结果具有显著性差异(P<0.05);研究组频繁碰壁1次(3.33%),低于对照组的3次(10.00%)、缝合层次错误2次(6.67%)、绕线方向错误3次(10.00%),研究组操作失误总发生率为1次(3.33%),低于对照组的10次(26.67%)(P<0.05)。结论对学员实施腹腔镜虚拟模拟培训系统,有利于提升腹腔镜手术缝合技能,降低操作失误率,提升缝合吻合质量。

Objective To compare and analyze the actual intervention situation of using laparoscopic virtual simulation training system in the training of laparoscopic surgical suture skill.Methods 60 resident physicians and interns who studied in the General Surgery Department of Friendship Hospital in Ili Kazakh Autonomous Prefecture from December 2022 to December 2023 were selected.According to different training methods,they were divided into the research group(with a laparoscopic virtual simulation training system)and the control group(with surgical suturing skill training).Each group consisted of 30 patients,and the suturing proficiency,anastomotic quality,and incidence of operational errors were compared between the two groups.Results The study group had higher scores in knotting quality,suture coordination,needle adjustment,and needle insertion after training compared to those of the control group,and the statistical results were statistically significant(P<0.05);After training,the study group was scored higher than the score of the control group in terms of incisional margin eversion,tight anastomosis,uniform needle spacing,and left-right symmetry,with significant differences in statistical results(P<0.05);The research group had only one frequent wall collision(3.33%),which was fewer than the control group's three frequent wall collision(10.00%),two errors in suture level(6.67%),and three errors in winding direction(10.00%).The total incidence of operational errors in the research group was one(3.33%),which was lower than the control group's ten(26.67%).The statistical results showed significant differences(P<0.05).Conclusion Implementing a laparoscopic virtual simulation training system for trainees is beneficial for improving their laparoscopic surgical suturing skills,reducing operational error rates,and improving the quality of suturing and anastomosis.It is worth promoting.
