跑台扰动训练对青年脑卒中偏瘫患者神经功能、平衡功能、步态及跌倒效能的影响

摘要 目的探究跑台扰动训练对青年脑卒中偏瘫患者神经功能、平衡功能、步态及跌倒效能的影响。方法纳入开封市中心医院康复治疗科2021年6月至2023年6月收治的96例青年脑卒中偏瘫患者作为研究对象,按照随机数字表分为两组,对照组48例给予渐进式核心肌群训练,观察组48例在对照组基础上联用跑台扰动训练,对比治疗后两组神经功能、平衡功能、步态及跌倒效能。结果经6周干预并随访1周可知,观察组美国国立卫生院卒中量表(NIHSS)评分(9.53±3.02)分相较于对照组(12.68±2.25)分低(P<0.05);Berg平衡量表(BBS)评分(41.67±7.13)分与Fuel-Meyer平衡量表评分(9.65±2.39)分相较于对照组BBS(30.44±6.21)分与Fuel-Meyer平衡量表(6.90±2.13)分高(P<0.05);跌倒效能量表(MFES)评分(99.08±8.94)分相较于对照组(82.02±9.66)分高,步速、步频及跨步长比率水平较对照组高,跌到指数(42.32±5.76)分相较于对照组(51.58±6.53)分低(P<0.05)。结论跑台扰动训练应用于青年脑卒中偏瘫患者,在促进神经功能及平衡功能恢复,纠正异常步态,减少跌倒率方面效果显著。 Objective To explore the effects of treadmill disturbance training on nerve function,balance function,gait and fall efficiency in young stroke patients with hemiplegia.Methods A total of 96 young stroke patients with hemiplegia admitted to the Rehabilitation Department of Kaifeng Central Hospital from June 2021 to June 2023 were included as the study objects,and were divided into two groups according to a random number table.48 cases in the control group were given progressive core muscle training,and 48 cases in the observation group were given combined treadmill disturbance training on the basis of the control group.Nerve function,balance function,gait and fall efficiency were compared between the two groups after treatment.Results After six weeks of intervention and one week of follow-up,the National Institutes of Health Stroke Scale(NIHSS)score of the observation group(9.53±3.02)was lower than that of the control group(12.68±2.25),P<0.05.Berg balance scale(BBS)scores(41.67±7.13)and Fuel-Meyer balance scale scores(9.65±2.39)were higher than those of the control group(30.44±6.21)and(6.90±2.13),P<0.05.The morse fall efficacy scale(MFES)score(99.08±8.94)was higher than that of the control group(82.02±9.66),the step speed,step frequency and stride length ratio were higher than those of the control group,and the fall index(42.32±5.76)was lower than that of the control group(51.58±6.53),P<0.05.Conclusion The application of treadmill disturbance training in young stroke patients with hemiplegia has significant effects on promoting the recovery of nerve function and balance function,correcting abnormal gait and reducing the fall rate.
