多参数CMR预测房颤患者术后复发因素评价

Evaluation of Factors Predicting Postoperative Recurrence in Patients with Atrial Fibrillation with Multi-parameter CMR

目的:心脏多参数磁共振成像对心房纤颤(AF)的患者进行分析,检测左房大小及心肌纤维化,探索预测房颤患者射频消融术后复发率的有效指标。方法:收集我院2020年1月至2023年4月期间,35例接受射频消融术后的AF患者,并将其分为两组:一组有复发症状,另一组没有。我们对这两组患者的CRM测量指标进行了比较,并研究了影响心房颤动患者早期复发的因素。结果:复发组CRM测量的左心房上下径[5.79±0.68cm]、左心房左右径[4.58±0.74cm],左心房容积[76.91±0.86ml],另一组相对应数值分别为[5.35±0.48cm、4.11±0.25cm、71.27±0.98ml],另外前组左心房心肌纤维化的比例(16/20,2/15)也显著提升;其中左心室功能前组LVEF的值为[103.5±18.93ml],LVEDV的值为[45.6±11.12ml],ESV的值为[58.3±18.93ml],SV的值为[45.6±11.12ml],后组而在其他指标上,两者的差异没有显著性的统计学差异。结论:CRM检查显示,左心房大小、心肌纤维化和左心室功能均可作为评估心脏功能的重要指标,但是前两项指标可以提供更准确的诊断信息,有统计学意义,左心室功能无显著影响。本研究重点探讨心房颤动射频消融技术治疗后心脏磁共振检查的评估。

Objective:To analyze patients with atrial fibrillation(AF)using multi parameter magnetic resonance imaging of the heart,detect left atrial size and myocardial fibrosis,and explore effective indicators for predicting the recurrence rate of atrial fibrllation patients after radiofrequency ablation.Methods:Between January 2020 and April 2023,we collected 35 AF patients who underwent radiofrequency ablation and divided them into two groups:one group had recurrent symptoms and the other group did not.We compared the CRM measurement indicators of these two groups of patients and investigated the factors affecting early recurrence in patients with atrial fibrllation.Results:The results showed that the upper and lower diameters of the left atrium[5.79±0.68cm],left and right diameters of the left atrium[4.58±0.74cm],and left atrial volume[76.91±0.86ml]were measured by CRM in the recurrent group.The corresponding values for the other group were[5.35±0.48cm,4.11±0.25cm,and 71.27±0.98ml],respectively.In addition,the proportion of left atrial myocardial fibrosis in the former group(16/20,2/15)was significantly increased;The LVEF value in the pre left ventricular function group was[103.5±18.93ml],the LVEDV value was[45.6±11.12ml],the ESV value was[58.3±18.93ml],and the SV value was[45.6±11.12ml].However,there was no significant statistical difference between the two in other indicators in the post group.Conclusion:CRM examination shows that left atrial size,myocardial fibrosis,and left ventricular function can all be important indicators for evaluating cardiac function.However,the first two indicators can provide more accurate diagnostic information and are statistically significant,with no significant impact on left ventricular function.
