温通刮痧联合针灸治疗寒湿型腰椎间盘突出症的临床疗效观察

Observation of the clinical efficacy of wentong scraping combined with acupuncture in the treatment cold⁃dampness type lumbar intervertebral disc herniation

目的:观察温通刮痧联合针灸治疗寒湿型腰椎间盘突出症的临床疗效。方法:选取我院中医科2022年5月至2023年5月148例寒湿型腰椎间盘突出患者为研究对象,使用随机数表法划分为两组,每组74例。对照组给予常规治疗,观察组在常规治疗基础上加予温通刮痧联合针灸治疗,治疗7 d。在治疗前及治疗后7 d分别采用日本骨科协会腰椎JOA评分、视觉模拟评分法(VAS)评分、Kolcaba舒适状况量表(GCQ)评分比较临床治疗效果,测量患者TGFβ1、TNFα、IL1β等炎症指标。结果:治疗后两组JOA得分提高(P<0.05),且观察组增长更为显著(P<0.05);治疗后观察组VAS评分明显下降、GCQ评分明显升高(P<0.05);临床疗效总有效率、显效效率,观察组均高于对照组(P<0.05)。治疗后,观察组TGFβ1、TNFα、IL1β水平均低于对照组(P<0.05)。结论:针对寒湿型腰椎间盘突出症,使用温通刮痧联合针灸治疗可以有效缓解症状,提高舒适状况及临床疗效。

Objective:In this study,our main focus was to evaluate the clinical efficacy of wentong scraping combined with acupuncture in the treatment of cold dampness type lumbar intervertebral disc herniation(LDH).Methods:A total of 148 patients with colddamp type lumbar disc herniation were admitted to our department from May 2022 to May 2023.They were randomly divided into two different groups.The first group was a control group and the second was an observation group.There were a total of 74 cases in each group.The control group was treated with conventional treatment,and the observation group was treated with acupuncture and infrared irradiation combined with Wentong scraping on the basis of conventional treatment for a total of 7 days.Results:After treatment,JOA scores in both groups had increased(P<0.05),but the increase was more significant in the observation group(P<0.05).In the observation group,VAS scores had significantly decreased and GCQ score had significantly increased,with statistical significance(P<0.05).Compared to the control group,the observation group had a higher effectiveness and cure rate.Following the intervention,both group’s levels of TGFβ1,TNFα,and IL1βwere considerably lower than they were prior to the therapy(P<0.05),with the observation group’s levels being significantly lower than those of the control group.Conclusion:Wentong scraping combined with acupuncture has proven to be effective in the treatment of colddampness type lumbar disc herniation,which helps to relieve pain symptoms and improve the clinical efficacy.
