地佐辛与异丙酚复合静脉麻醉在子宫黏膜下肌瘤患者中的应用分析

Analysis of Compound Intravenous Anesthesia with Dezocine and Propofol in Patients with Submucous Uterine Fibroids

目的分析子宫黏膜下肌瘤(Submucous Uterine Myoma,SUM)患者采用地佐辛与异丙酚复合静脉麻醉的效果。方法随机选取2021年9月—2023年8月明溪县总医院收治的SUM手术患者70例作为研究对象,按照随机数表法分为两组,各35例。两组均在宫腔镜下进行子宫肌瘤切除术,对照组患者使用异丙酚静脉麻醉,研究组患者使用地佐辛与异丙酚复合静脉麻醉。对比两组术中不同时间点心率与平均动脉压指标、麻醉相关指标、术后视觉模拟评分量表(Visual Analogue Scale,VAS)评分与不良反应情况。结果研究组在麻醉诱导后、扩宫时的心率、平均动脉压低于对照组,差异有统计学意义(P均<0.05)。两组麻醉起效时间、呼吸恢复时间、意识恢复时间、睁眼时间对比,差异无统计学意义(P均>0.05)。术后研究组5 min、60 min、120 min时的VAS评分低于对照组,差异有统计学意义(P均<0.05)。研究组术后不良反应发生率为2.86%,低于对照组的22.86%,差异有统计学意义(χ^(2)=4.590,P<0.05)。结论地佐辛与异丙酚复合静脉麻醉能够保持SUM患者术中血流动力学的稳定性,提高术后镇痛效果,降低不良反应风险。

Objective To analyze the effect of compound intravenous anesthesia with dezocine and propofol in patients with submucous uterine myoma(SUM).Methods A total of 70 SUM surgery patients admitted to Mingxi County General Hospital from September 2021 to August 2023 were randomly selected as study objects and divided into two groups of 35 patients each according to the random number table method.Both groups underwent myomectomy under hysteroscopy,and patients in the control group were anesthetized with isoproterenol intravenously,while patients in the study group were anesthetized with dizocin and isoproterenol combined intravenously.The heart rate and mean arterial pressure indexes at different times during the operation,anesthesia-related indexes,postoperative Visual Analogue Scale(VAS)scores and adverse reactions were compared between the two groups.Results The heart rate and mean arterial pressure of the study group after induction of anesthesia and during dilatation were lower than those of the control group,and the differences were statistically significant(all P<0.05).Comparison of anesthesia onset time,respiratory recovery time,consciousness recovery time,and eye opening time between the two groups showed no statistically significant differences(all P>0.05).The postoperative VAS scores at 5 min,60 min and 120 min in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The incidence rate of postoperative adverse reactions in the study group was 2.86%,lower than 22.86% in the control group,and the difference was statistically significant(χ^(2)=4.590,P<0.05).Conclusion Compound intravenous anesthesia with dezocine and propofol can maintain the stability of intraoperative hemodynamics in patients with SUM,improve post operative analgesic effect,and reduce the risk of adverse reactions.
