富血小板血浆治疗斑秃疗效与安全性的Meta分析

A Meta-analysis of the efficacy and safety of platelet-rich plasma in treatment of alopecia areata

目的系统评价富血小板血浆(PRP)治疗斑秃(AA)的临床疗效和安全性。方法检索中国知网、万方医学网、维普网、Sinomed数据库、PubMed数据库、EMBASE数据库、Cochrane Library数据库和Web of Science数据库,检索时间为数据库建库至2023年4月,纳入PRP治疗AA的相关临床研究,研究指标包含脱发严重程度工具(SALT)评分和SALT 6分法评分。系统评价PRP治疗AA的疗效及安全性。结果共纳入7项研究进行Meta分析。PRP治疗后,患者SALT评分显著下降(P<0.05)。PRP治疗组和类固醇治疗组患者的SALT评分均下降,但两组评分差异无统计学意义[SMD(95%CI)=-0.41(-1.22~0.40),I^(2)=81.00%,P=0.33]。有5项研究提及了PRP治疗AA过程中不良反应的发生情况,主要表现为短暂的注射部位疼痛、局部红斑、烧灼感、水肿等,短时间内均可恢复,AA患者对PRP治疗的耐受性普遍良好。结论PRP治疗AA是安全有效的。

Objective To systematically evaluate the clinical efficacy and safety of platelet-rich plasma(PRP)in treatment of alopecia areata(AA).Methods China National Knowledge Infrastructure(CNKI),Wanfang medical online,VIP network,Sinomed database,PubMed database,EMBASE database,Cochrane Library database and Web of Science database were searched.During the period from the establishment of the databases to April 2023,the papers were searched and the relevant clinical studies on the treatment of AA with PRP were included,and the evaluation indicators included study indicators involving the Severity of Alopecia Tool(SALT)score or the SALT 6-point score.The efficacy and safety of PRP in treatment of AA were systematically evaluated.Results A total of 7 studies were included for Meta-analysis.After PRP treatment,the SALT score of the patients decreased significantly(P<0.05).SALT score decreased in both PRP treatment group and steroid treatment group,but there was no statistically significant difference in the SALT score between the two groups[SMD(95%CI)=-0.41(-1.22-0.40),I^(2)=81.00%,P=0.33].Five studies had mentioned the occurrence of adverse reactions during the treatment of AA with PRP.These adverse reactions were mainly manifested as transient injection site pain,local erythema,burning sensation and edema,which could be recovered within a short time,and AA patients were generally well tolerated for PRP treatment.Conclusion PRP is safe and effective in treating AA.
