患者参与控制联合前瞻性干预对急性髓系白血病经外周静脉穿刺的中心静脉导管置管化疗患者的效果

Effect of patient participation control combined with prospective intervention on chemotherapy patients with acute myeloid leukemia undergoing peripherally inserted central venous catheters catheterization

目的 探讨患者参与控制联合前瞻性干预对急性髓系白血病(AML)经外周静脉穿刺的中心静脉导管(PICC)置管化疗患者的效果。方法 前瞻性纳入2022年1月至2023年4月上海交通大学医学院附属新华医院100例AML行PICC置管化疗患者,根据随机数字表法分为两组,各50例。对照组实施前瞻性干预,观察组实施患者参与控制联合前瞻性干预,干预6个月。比较两组置管前及置管6个月时自我护理能力、心理状态,记录两组并发症发生情况。结果 置管6个月时,两组自我护理能力量表评分均较置管前升高,且观察组高于对照组(P<0.05)。置管6个月时,两组焦虑自评量表及抑郁自评量表评分均较置管前降低,且观察组低于对照组(P<0.05)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论 患者参与控制联合前瞻性干预可提高AML行PICC置管化疗患者自我护理能力,减轻负性情绪。

Objective To explore the effect of patient participation control combined with prospective intervention on chemotherapy patients with acute myeloid leukemia(AML)undergoing peripherally inserted central venous catheters(PICC)catheterization.Methods A prospective study was conducted on 100 chemotherapy patients with AML undergoing PICC catheterization at Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2022 to April 2023.They were divided into two groups by random number table method,with 50 patients in each group.Control group was implemented with prospective intervention,while observation group received patient participation control combined with prospective intervention,with intervention lasted for six months.The self-care ability,psychological status between two groups were compared before catheterization and six months of catheterization.Results At six months of catheterization,exercise of self-care agency scale scores in two groups were higher than those before catheterization,and those of observation group were higher than those of control group(P<0.05).At six months of catheterization,self-rating anxiety scale and self-rating depression scale scores in two groups were lower than those before catheterization,and those of observation group were lower than those of control group(P<0.05).There was no significant difference in total incidence of complications between two groups(P>0.05).Conclusion Patient participation control combined with prospective intervention can improve the self-care ability of chemotherapy patients with AML undergoing PICC catheterization,alleviate negative emotions.
