通络益肾方联合非奈利酮治疗糖尿病肾病的临床效果

Clinical effect of Tongluo Yishen prescription combined with finerenone in the treatment of diabetic kidney disease

目的研究通络益肾方联合非奈利酮治疗糖尿病肾病(DKD)的临床效果。方法选择2023年1月至5月在我院诊治的40例DKD患者作为研究对象,按照随机数字表法将其分为对照组和观察组,每组20例。对照组给予非奈利酮治疗,观察组采用通络益肾方联合非奈利酮治疗。比较两组的肾功能指标、炎症因子、氧化应激指标、临床疗效及用药安全性。结果治疗后,观察组的血尿素氮(BUN)、肌酐(SCr)、尿微量白蛋白(mALB)、尿白蛋白与肌酐比值(UACR)、24 h尿蛋白定量(UP-24 h)水平明显低于对照组,估算肾小球滤过率(eGFR)显著高于对照组(P<0.05)。治疗后,观察组的白细胞介素-6(IL-6)、丙二醛(MDA)、超敏C反应蛋白(hs-CRP)水平低于对照组,超氧化物歧化酶(SOD)水平高于对照组(P<0.05)。观察组的治疗总有效率显著高于对照组(P<0.05)。结论通络益肾方联合非奈利酮治疗DKD患者可以有效控制尿蛋白、减轻炎性反应、改善氧化应激水平。

Objective To study the clinical effect of Tongluo Yishen prescription combined with finerenone in the treatment of diabetic kidney disease(DKD).Methods A total of 40 patients with DKD diagnosed and treated in our hospital from January to May 2023 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,with 20 cases in each group.The control group was treated with finerenone,and the observation group was treated with Tongluo Yishen prescription combined with finerenone.The renal function indexes,inflammation factors,oxidative stress indexes,clinical efficacy and medication safety were compared between the two groups.Results After treatment,the levels of blood urea nitrogen(BUN),serum creatinine(SCr),microalbuminuria(mALB),urine albumin-to-creatinine ratio(UACR)and 24-h urinary protein excretion(UP-24 h)in the observation group were significantly lower than those in the control group,and the estimated glomerular filtration rate(eGFR)was significantly higher than that of the control group(P<0.05).After treatment,the levels of interleukin-6(IL-6),malondialdehyde(MDA)and high-sensitive C-reactive protein(hs-CRP)in the observation group were lower than those in the control group,and the level of superoxide dismutase(SOD)was higher than that in the control group(P<0.05).The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Tongluo Yishen prescription combined with finerenone in the treatment of patients with DKD can effectively control urinary protein,reduce inflammatory reaction,and improve oxidative stress level.
