老年食管癌患者围术期心理疏导需求程度的影响因素分析

Influencing Factors of Perioperative Psychological Counseling Demand in Elderly Patients with Esophageal Cancer

目的:分析老年食管癌患者围术期心理疏导需求程度的影响因素。方法:选取2020年1月—2022年6月于南京医科大学附属无锡人民医院实施微创食管癌切除术的老年食管癌患者150例作为研究对象,统计其临床资料,分析老年食管癌患者围术期心理疏导需求程度的影响因素。结果:不同年龄、癌症分期、文化程度、社会支持程度老年食管癌患者围术期心理疏导需求程度比较,差异有统计学意义(P<0.001)。年龄>75岁、癌症分期≥Ⅱ期、文化程度初中及以下、社会支持程度低下是老年食管癌患者围术期心理疏导需求程度的独立影响因素(P<0.001)。结论:年龄>75岁、癌症分期≥Ⅱ期、文化程度初中及以下、社会支持程度低下是老年食管癌患者围术期心理疏导需求程度的独立影响因素,临床应以多种方式对患者负性情绪进行疏泄引导,满足老年食管癌患者围术期的心理疏导需求。

Objective:To analyze the factors influencing the degree of psychological counseling demand in the perioperative period of elderly patients with esophageal cancer.Methods:A total of one hundred and fifty elderly patients with esophageal cancer who underwent minimally invasive resection for esophageal cancer in Wuxi People's Hospital Affiliated to Nanjing Medical University from January 2020 to June 2022 were selected as the study objects.The clinical data were analyzed and the factors influencing the degree of psychological counseling in the perioperative period of elderly patients with esophageal cancer were analyzed.Results:There were statistically significant differences in perioperative psychological counseling needs among elderly patients with esophageal cancer in different age,cancer stage,education level and social support level(P<0.001);age>75 years old,cancer stage≥Ⅱ,education level of junior high school or below,and low degree of social support were the independent influencing factors of perioperative psychological counseling need of elderly patients with esophageal cancer(P<0.001).Conclusion:Age>75 years old,cancer stage≥Ⅱ,education level of junior high school or below,and low degree of social support were the independent factors influencing the degree of psychological counseling need in the perioperative period of elderly patients with esophageal cancer.In clinical practice,patients'negative emotions should be relieved and guided in various ways to meet the psychological counseling needs of elderly patients with esophageal cancer in perioperative period.
