催乳汤联合乳房局部按摩治疗产后缺乳患者的回顾性研究

A Retrospective Study of Cuiru Decoction(催乳汤)Combined with Local Breast Tuina in Treatment of Postpartum Patients with Lack of Breast

目的探讨催乳汤联合乳房局部按摩治疗产后缺乳患者的临床效果及对其激素水平的影响。方法回顾性选取2020年2月—2022年1月郑州市妇幼保健院收治的产后缺乳患者60例,按照治疗方式的不同将其分为观察组(32例)和对照组(28例)。两组患者均接受母乳喂养指导,对照组实施乳房局部按摩治疗,观察组在对照组基础上联合催乳汤治疗。两组患者均连续治疗1周,并随访至产后42 d。比较两组治疗后的治疗效果、泌乳量评分,泌乳始动时间,产后42 d纯母乳喂养情况,治疗前后的临床症状积分、泌乳量、补乳量、乳房充盈程度、激素水平和安全性。结果治疗后观察组较高的临床总有效率;观察组治疗后泌乳量评分高于对照组,泌乳始动时间短于对照组;与治疗前比较,治疗后两组患者主症评分、次症评分、补乳量、血清泌乳素水平均降低,观察组低于对照组;治疗后两组患者泌乳量、乳房充盈程度、血清孕酮(progesterone,P)、雌二醇(estradiol,E_(2))水平与治疗前比较均升高。组间进行比较,观察组水平较高,差异有统计学意义(P<0.05)。两组产后42 d纯母乳喂养率经比较,差异无统计学意义(P>0.05)。治疗期间,两组患者均未发生不良反应情况。结论催乳汤联合乳房局部按摩治疗产后缺乳患者可有效缓解临床症状,改善乳房充盈程度及泌乳情况,调节激素水平,疗效显著且安全性高。

Objective To investigate the clinical effect of Cuiru Decoction(催乳汤)combined with local breast Tuina in the treatment of postpartum patients with lack of breast and its influence on hormone levels.Methods From February 2020 to January 2022,60 patients with postpartum lack of breast admitted to Zhengzhou Maternal and Child Health Hospital were retrospectively selected and divided into the observation group(32 cases)and the control group(28 cases)according to different treatment methods.Both groups received breastfeeding instruction.The control group was treated with local breast Tuina,and the observation group was treated with Cuiru Decoction on the basis of the control group.Both groups were treated for 1 week and followed up to 42 days postpartum.The treatment effect,lactation score,lactation onset time,exclusive breastfeeding at 42 days after delivery,clinical symptom score,lactation volume,milk supplement volume,breast filling degree,hormone level before and after treatment and safety during treatment were compared between the two groups.Results The total effective rate of the observation group was higher.The lactation volume score was higher and the lactation onset time was shorter in the observation group.Compared with those before treatment,the main symptom score,secondary symptom score,milk supplement amount and serum prolactin level of the two groups decreased after treatment,and those of the observation group were lower.Compared with those before treatment,lactation volume,breast filling degree,serum levels of progesterone(P)and estradiol(E_(2))in the two groups increased after treatment,and those of the observation group were higher,and the differences were statistically significant(P<0.05).There was no significant difference in the rate of exclusive breastfeeding at 42 days after delivery between the two groups(P>0.05).During treatment,no adverse reactions occurred in the two groups.Conclusion Cuiru Decoction combined with local breast Tuina can effectively relieve the clinical symptoms,improve the degree of breast filling and lactation,regulate the hormone level,and has significant efficacy and high safety in the treatment of postpartum patients with lack of breast.
