自拟促愈方与康复新液治疗肛肠术后难愈性创面临床疗效及对ECM的影响

Comparison of Clinical Efficacy and Influence on ECM Components of Self-made Cuyu Decoction(自拟促愈方)and Kangfuxin Liquid(康复新液)in Treatment of Refractory Wound after Anorectal Surgery

目的探究自拟促愈方与康复新液治疗肛肠术后难愈性创面临床疗效及对细胞外基质(extracellular matrix,ECM)的影响。方法选取商丘市中医院肛肠科2021年4月—2022年6月收治的90例肛肠术后创面难愈患者,按照随机数字表法分为对照组与治疗组,各45例,对照组采用康复新液治疗,治疗组采用自拟促愈方治疗,比较3个疗程后两组治疗总有效率、创面愈合及住院时间、创面疼痛程度[视觉模拟评分法(visual analogue scale,VAS)]ECM含量。结果治疗组总有效率82.22%(37/45),显著高于对照组62.22%(28/45)(P<0.05)。治疗组的创面愈合及住院时间明显短于对照组(P<0.05)。治疗后,两组VAS评分均明显下降(P<0.05),且治疗组VAS评分明显低于对照组(P<0.05),治疗组患者疼痛减轻明显优于对照组;治疗14 d后,两组ECM含量明显增多(P<0.05),且治疗组明显高于对照组(P<0.05)。结论自拟促愈方较康复新液治疗肛肠术后难愈性创面的临床疗效较好。可明显缓解患者临床病症、减轻患者痛苦。

Objective To compare the clinical efficacy and influence on extracellular matrix(ECM)components of Self-made Cuyu Decoction(自拟促愈方)and Kangfuxin Liquid(康复新液)treating refractory wound after anorectal surgery.Methods Ninety patients with refractory wound after surgery in Anorectal Surgery of Shangqiu Hospital of Traditional Chinese Medicine from April 2021 to June 2022 were enrolled,and classified into two groups according to random number table methods,each with 45 cases.The control group received Kangfuxin Liquid,while the treatment group received Self-made Cuyu Decoction.After three courses of treatment,the total effective rate,wound healing time,length of stay,visual analogue scale(VAS)and ECM components of two groups were compared.Results The total efficacy rate was 82.22%(37/45)in the treatment group,which was notably higher than 62.22%(28/45)in the control group with statistical difference(P<0.05).The treatment group had decreased time of wound healing and hospital stay than the control group,with statistical difference(P<0.05).VAS score showed a decrease in both groups after treatment(P<0.05),and the treatment group scored lower on VAS than the control group after treatment(P<0.05),denoting that the pain alleviation was better in the treatment group than that in the control group.ECM components showed an increase in both groups after treatment,and the treatment group had higher levels of ECM than the control group(P<0.05).Conclusion Compared with Kangfuxin Liquid therapy,the application of Self-made Cuyu Decoction in the treatment of refractory wound after anorectal surgery achieves better clinical efficacy,which can effectively improve the clinical symptoms and alleviate the pain in patients.
