针刺配合穴位注射治疗颈椎病患者的临床疗效及症状改善情况

Clinical Efficacy and Symptom Improvement of Acupuncture Combined with Acupoint Injection in Treatment of Patients with Cervical Spondylosis

目的分析针刺配合穴位注射治疗颈椎病患者的临床疗效及症状改善情况。方法选取2020年12月—2022年12月中国人民解放军海军安庆医院收治的80例混合型颈椎病患者为观察对象,采用随机数字表法分为针刺组和穴位注射组各40例,针刺组采用针刺穴位进行治疗,穴位注射组采用针刺配合穴位注射丹参注射液进行治疗,比较两组治疗后的临床疗效、症状改善情况、颈椎活动度、血液流变学指标水平、生活质量评分,并进行安全性评价。结果治疗后,穴位注射组临床总有效率(92.50%,37/40)显著高于针刺组(75.00%,30/40)(P<0.05);两组中医证候积分均显著降低(P<0.05),颈椎整体症状积分均显著升高(P<0.05),且组间各积分差异有统计学意义(P<0.05);两组颈椎前屈、后伸、左侧屈、右侧屈活动度均显著升高(P<0.05),且穴位注射组各活动度明显高于针刺组(P<0.05);穴位注射组红细胞聚集指数、红细胞压积、血浆黏度均显著下降(P<0.05),且血浆黏度明显低于针刺组(P<0.05);两组主观症状、临床体征、适应能力评分及总分均显著升高(P<0.05),且穴位注射组分数均明显高于针刺组(P<0.05);治疗期间两组均未出现明显不良反应。结论应用针刺配合穴位注射有助于提高混合型颈椎病患者临床疗效,改善临床症状、颈椎活动度及生活质量,且可调节血液流变学状态,安全可靠,值得临床推广应用。

Objective To analyze the clinical efficacy and symptom improvement of acupuncture combined with acupoint injection in the treatment of patients with cervical spondylosis.Methods Eighty patients with mixed cervical spondylosis admitted to the hospital from December 2020 to December 2022 were selected as the observation subjects and were divided into acupuncture group and acupoint injection group by the random number table method,with 40 cases in each group.The acupuncture group was treated with acupuncture,and the acupoint injection group was given acupuncture combined with acupoint injection of Danshen Injection.The clinical efficacy,symptom improvement,cervical range of motion,hemorheological indicators and quality of life scores were compared between the two groups after treatment,and the safety was evaluated.Results After treatment,the total clinical effective rate in the acupoint injection group(92.50%,37/40)was significantly higher than that in the acupuncture group(75.00%,30/40)(P<0.05).The TCM symptom score was significantly decreased in both groups(P<0.05)while the score of overall cervical symptom was significantly increased(P<0.05),and the scores were significantly different between the groups(P<0.05).The ranges of motions of cervical flexion,rear protraction,left flexion and right flexion in both groups were significantly enhanced(P<0.05),and the ranges of motions in the acupoint injection group were significantly higher than those in the acupuncture group(P<0.05).The red blood cell aggregation index,hematocrit and plasma viscosity in the acupoint injection group were significantly declined(P<0.05),and the plasma viscosity was significantly lower than that in the acupuncture group(P<0.05).The scores of subjective symptom,clinical sign and adaptive capacity and total score were significantly increased in both groups(P<0.05),and the scores in the acupoint injection group were significantly higher than those in the acupuncture group(P<0.05).During treatment,there were no obvious adverse reactions in both groups.Conclusion Acupuncture combined with acupoint injection can help to enhance the clinical efficacy,improve the clinical symptom,cervical range of motion and quality of life and regulate the hemorheology in patients with mixed cervical spondylosis.It is safe and reliable and worthy of clinical promotion and application.
