彩色多普勒超声诊断在临床乳腺癌腋窝淋巴结清扫中的指导意义

The Guiding Significance of Color Doppler Ultrasonography in Axillary Lymph Node Dissection of Clinical Breast Cancer

目的:以乳腺癌腋窝淋巴结患者为观察对象,以彩色多普勒超声为研究方法,就该检查方法在该疾病中的诊断价值进行深入分析与探讨。方法:选取2022年1月~2023年6月本院收治的100例疑似乳腺癌腋窝淋巴结患者为观察对象,患者均以手术病理检查结果作为金标准,同时采用彩色多普勒超声检查,就该检查方法的诊断效能及在良恶性结节中的诊断差异进行对比分析。结果:彩色多普勒超声检查与手术病理检查金标准在乳腺癌腋窝淋巴结阳性与阴性鉴别诊断的一致性良好(Kappa=0.758)。彩色多普勒超声检查与手术病理检查金标准针对乳腺癌腋窝淋巴结良恶性鉴别诊断的一致性良好(Kappa=0.887)。彩色多普勒超声检查结果显示,良性结节与恶性结节的声像特征均存在显著差异(P<0.05)。彩色多普勒超声检查结果显示,良性结节与恶性结节的血流信号分布情况和血流分级情况均存在显著差异(P<0.05)。结论:彩色多普勒超声在乳腺癌腋窝淋巴结具有较高的诊断价值,能进一步提高患者良性与恶性结节的检出率,为临床清扫淋巴结提供准确可靠的指导意见。

Objective:The value of color Doppler ultrasonography in the diagnosis of axillary lymph nodes with breast cancer was analyzed and discussed.Methods:The observation objects in this paper were 100 patients with axillary lymph nodes suspected of breast cancer admitted to our hospital from January 2022 to June 2023.Therefore,the results of surgical pathological examination were taken as the gold standard for all patients,and color Doppler ultrasound was used to compare and analyze the diagnostic efficacy of this examination method and the difference in diagnosis of benign and malignant nodules.Results:The gold standard of color Doppler ultrasonography and surgical pathology in the differential diagnosis of positive and negative axillary lymph nodes of breast cancer was in good agreement(Kappa=0.758).There was good agreement between color Doppler ultrasonography and the gold standard of surgical pathology in the differential diagnosis of benign and malignant axillary lymph nodes of breast cancer(Kappa=0.887).The results of color Doppler ultrasonography showed that there were significant differences between benign and malignant nodules(P<0.05).The results of color Doppler ultrasound showed that there were significant differences in blood flow signal distribution and blood flow grading between benign and malignant nodules(P<0.05).Conclusion:Color Doppler ultrasound has high diagnostic value in axillary lymph nodes of breast cancer,which can further improve the detection rate of benign and malignant nodules,and provide accurate and reliable guidance for clinical lymph node dissection.
