舒筋通络颗粒治疗气滞血瘀型神经根型颈椎病的电生理评价与效果分析

Electrophysiological evaluation and effect analysis of Shujin Tongluo Granules in the treatment of cervical spondylotic radiculopathy with stagnation of qi and blood stasis type

目的 观察舒筋通络颗粒对气滞血瘀型神经根型颈椎病(CSR)电生理指标的影响。方法 选取2021年9月至2022年8月石家庄市中医院骨科门诊CSR患者64例,按照随机数字表法将其分为治疗组和对照组,每组32例。对照组服用甲钴胺片,治疗组在对照组基础上服用舒筋通络颗粒,疗程均为4周。比较两组治疗前后F波潜伏期、F波传导速度、F波出现率、视觉模拟评分法(VAS)评分、颈椎日本骨科学会(JOA)评分、中医证候评分和临床疗效。结果 对照组有1例患者脱落,最终纳入31例,治疗组有2例患者脱落,最终纳入30例。治疗后,两组正中神经、尺神经F波潜伏期较治疗前缩短,且治疗组低于对照组(P<0.05)。治疗后,两组正中神经、尺神经F波传导速度较治疗前提高,且治疗组高于对照组(P<0.05)。治疗后,两组正中神经、尺神经F波出现率较治疗前均提高,且治疗组高于对照组(P<0.05)。治疗后,两组颈椎VAS、中医证候评分较治疗前降低,且治疗组低于对照组(P<0.05)。治疗后,两组颈椎JOA评分较治疗前升高,且治疗组高于对照组(P<0.05)。治疗组临床疗效优于对照组(P<0.05)。结论 舒筋通络颗粒治疗气滞血瘀型CSR可以改善神经功能,缓解疼痛,临床效果确切。

Objective To observe the effect of Shujin Tongluo Granules on electrophysiological indexes of cervical spondylotic radiculopathy(CSR) with stagnation of qi and blood stasis type.Methods A total of 64 patients with CSR in the Department of Orthopedic Outpatient,Shijiazhuang Hospital of Traditional Chinese Medicine from September 2021 to August 2022 were selected and they were divided into treatment group and control group according to random number table method,with 32 cases in each group.The control group took Methylcobalamin Tablets,and the treatment group took Shujin Tongluo Granules on the basis of the control group.The course of treatment was four weeks.F-wave latency,F-wave conduction velocity,F-wave occurrence rate,visual analogue scale(VAS) score,cervical spine Japanese Orthopaedic Society(JOA) score,traditional Chinese medicine syndrome score before and after treatment and clinical efficacy were compared.Results In the control group,one patient fell out and 31 patients were eventually included,while in the treatment group,two patients fell out and 30 patients were eventually included.After treatment,F-wave latency of median nerve and ulnar nerve in both groups was shorter than that before treatment,and that in the treatment group was lower than that in the control group(P<0.05).After treatment,the F-wave conduction velocity of median nerve and ulnar nerve in both groups was higher than that before treatment,and that in treatment group was higher than that in control group(P<0.05).After treatment,the cervical VAS and traditional Chinese medicine syndrome scores of the two groups were lower than those before treatment,and those of the treatment group were lower than those of the control group(P<0.05).After treatment,the cervical spine JOA score of the two groups was higher than that before treatment,and that in the treatment group was higher than that in the control group(P<0.05).The clinical effect of the treatment group was better than that of the control group(P<0.05).Conclusion Shujin Tongluo Granules can improve nerve function and relieve pain in the treatment of CSR with stagnation of qi and blood stasis type,and the clinical effect is accurate.
