基于SFIC理论的公立医院门诊应急联动影响因素及优化策略研究

Research on influencing factors and optimization strategies of public hospital outpatient emergency linkage based on SFIC theory

目的 探索公立医院门诊应急联动的影响因素,为提升门诊应急联动效果、助推门诊平稳运行提供参考。方法 先以SFIC理论为基础,构建公立医院门诊应急联动影响因素问卷;于2023年3月至4月对筛选出的25名专家发放问卷;最后运用决策试验和评价实验室法分析问卷数据,探索影响应急联动因素的影响度、被影响度、中心度与原因度。结果 本研究结合理论模型与文献分析构建了5个维度21个因素的指标评价体系,并识别出10个关键因素,包括院内环境、组织结构、指挥领导、可操作性、规范全面、合作基础、沟通渠道、建立信任、积极投入与风险共担。结论 从夯实联动初始条件、加强应急制度建设、提升领导指挥能力方面入手可提升公立医院门诊应急联动效果。

Objective To explore the influencing factors of outpatient emergency linkage in public hospitals,and to provide reference for improving the effect of outpatient emergency linkage and promoting the smooth operation of outpatient.Methods Based on SFIC theory,the questionnaire of influencing factors of public hospital outpatient emergency linkage was constructed.Questionnaires were distributed to 25 selected experts from March to April 2023;finally,decision making trial and evaluation laboratory method was used to analyze the questionnaire data to explore the impact degree,affected degree,center degree and cause degree of the factors affecting the emergency linkage.Results This study combined theoretical model and literature analysis to build an index evaluation system with 21 factors in five dimensions,and identified ten key factors,including hospital environment,organizational structure,command and leadership,operability,normative comprehensiveness,cooperation basis,communication channels,trust building,active input,and risk sharing.Conclusion The effect of public hospital outpatient emergency linkage can be improved by consolidating the initial conditions of linkage,strengthening the construction of emergency system and improving the leadership and command ability.
