经超声引导股动脉穿刺建立介入治疗入路在肥胖人群中的应用效果观察

摘要 目的:观察经超声引导股动脉穿刺建立介入治疗入路在肥胖人群中的应用效果。方法:选取2020年12月—2021年12月威海市中心医院收治的60例行股动脉穿刺建立介入治疗入路的肥胖患者为研究对象,采用随机数字表法分为经超声引导下穿刺组与触诊法穿刺组各30例,比较两组操作次数以及操作时间。结果:60例患者均成功进行股动脉穿刺建立介入通路。其中触诊法穿刺组操作次数为1(1.0,2.0)次,操作时间为(10.5±4.6)min;经超声引导下股动脉穿刺组操作次数为1(1.0,1.0)次,操作时间为(7.0±2.3)min。术后观察触诊法穿刺的30例中有1例患者出现穿刺部位血肿。两组均未出现穿刺部位血管狭窄闭塞、动静脉瘘或假性动脉瘤等并发症。超声引导下穿刺组的穿刺操作次数少于触诊法穿刺组,操作时间短于触诊法穿刺组,差异有统计学意义(P<0.05)。结论:在肥胖人群中采用超声引导下股动脉穿刺建立介入治疗入路安全、有效,可以节约时间,降低穿刺相关并发症的发生率。 Objective To exploring the application effect of ultrasound-guided femoral artery puncture for establishing interventional therapy access in the obese population.Methods A total of 60 obese individuals underwent femoral artery puncture to establish interventional treatment approach admitted to Weihai Central Hospital from December 2020 to December 2021 were selected as the study subjects.They were divided into two groups using a random number table method:the ultrasound-guided puncture group and the palpation-guided puncture group,with 30 cases in each group.The number of procedures and the duration of the procedures were compared between the two groups.Results All 60 patients successfully underwent femoral artery puncture to establish interventional access.In the palpation-guided puncture group,the number of procedures were 1(1.0,2.0)times,and the duration of the procedures were(10.5±4.6)minutes.In the ultrasound-guided femoral artery puncture group,the number of procedures were 1(1.0,1.0)times,and the duration of the procedures was(7.0±2.3)minutes.Postoperatively,among the 30 cases in the palpation-guided puncture group,one patient developed hematoma at the puncture site.No other complications such as vascular stenosis,occlusion,arteriovenous fistula,or pseudoaneurysm occurred in the other patients.The number of puncture procedures in the ultrasound-guided group was fewer than that in the palpation-guided group,and the duration of procedures was shorter than that in the palpation-guided group,with statistical significance(P<0.05).Conclusions It is safe and effective to establish an interventional approach by ultrasound-guided femoral artery puncture in obese people,which can save time and reduce the incidence of puncture related complications.
