多学科协作模式在高龄老年髋部骨折中应用的临床疗效评价

Evaluation of clinical efficacy of multidisciplinary collaborative modelin elderly patients with hip fractures

目的评估骨科与老年科共管结合的多学科协作诊疗模式对高龄老年髋部骨折的临床应用效果。方法选取2021年3月至2023年2月该院收治的180例高龄老年髋部骨折患者为研究对象,并将其分为对照组和观察组,各90例。对照组患者采用传统的单骨科管理治疗模式,观察组患者采用骨科与老年科共管结合的多学科协作模式进行诊断和治疗。比较2组患者的术前等待时间、术后进食时间、术后首次下床时间、伤后住院时间和围手术期并发症发生率等指标。结果观察组患者术前等待时间、术后进食时间、术后首次下床时间、伤后住院时间较对照组显著缩短,且观察组术后并发症如肺炎、泌尿系感染和下肢深静脉血栓形成的发生率显著低于对照组,差异均有统计学意义(P<0.05)。结论骨科与老年科共管结合的多学科协作诊疗模式能够更快速地评估高龄老年髋部骨折患者的术前状况并进行早期干预和治疗,缩短术前等待时间,降低围手术期病死率,缩短住院时间,并减少术后并发症发生。

Objective To evaluate the clinical application effect of multidisciplinary collaborative diagnosis and treatment model combined with orthopedics and geriatrics in the treatment of hip fracture in the elderly.Methods A total of 180 elderly patients with hip fracture admitted to this hospital from March 2021 to February 2023 were selected as the study objects and divided into the control group and the observation group,with 90 cases in each group.The participants in the control group were administered the traditional single-orthopedic treatment,while the observation group underwent the multidisciplinary collaborative diagnosis and treatment mode combined with the orthopaedics and geriatrics.The preoperative waiting time,postoperative feeding time,first time of getting out of bed,postoperative hospital stay and perioperative complication rate were compared between the two groups.Results The preoperative waiting time,postoperative feeding time,first time of getting out of bed,postoperative hospital stay in the observation group were significantly shorter than those in the control group,and the incidence of postoperative complications such as pneumonia,urinary system infection and deep venous thrombosis of lower limbs in the observation group were significantly lower than those in the control group,with statistical significance(P<0.05).Conclusion The multidisciplinary collaborative diagnosis and treatment mode combined with the orthopaedics and geriatrics can more quickly assess the preoperative status of elderly patients with hip fracture and carry out early intervention and treatment,shorten the preoperative waiting time,reduce perioperative mortality,shorten hospital stay,and reduce postoperative complications,which conforms to the concept of rapid rehabilitation.
