我国肿瘤患者社会疏离现状及影响因素的系统评价

Current status and influencing factors of social isolation among cancer patients in China: A systematic review

目的系统评价我国肿瘤患者社会疏离水平及影响因素。方法计算机检索中国知网、万方、维普、中国生物医学文献数据库、PubMed、Embase、Web of Science、CINAHL等数据库,检索有关肿瘤患者社会疏离现状及(或)影响因素的研究,检索时限为建库至2023年7月31日。由2名研究者独立筛选文献、提取资料并对纳入研究的偏倚风险进行评价,采用Stata 16.0软件对患者社会疏离得分进行Meta分析,对影响因素进行描述性分析。结果共纳入21篇文献,包括5031位肿瘤患者。Meta分析结果显示,肿瘤患者的社会疏离评分为40.60[95%CI(38.61,42.58)]分。影响其社会疏离水平的因素包括人口学因素(年龄、性别、婚姻状况)以及其他相关因素(疾病分期、社会支持)。结论我国肿瘤患者的社会疏离处于中等偏上水平,亟须进一步的干预和管理。建议重点关注女性、受教育程度低、月收入低以及肿瘤分期Ⅲ期以上的患者,制定针对性的干预措施,改善肿瘤患者的社会心理问题,促进其积极融入社会。

Objective To systematically evaluate the level and influencing factors of social alienation among cancer patients in China.Methods CNKI,WanFang,VIP,CBM,PubMed,Embase,Web of Science,CINAHL,and other databases were searched to retrieve studies on the status of social alienation and/or influencing factors of cancer patients.The retrieval time was from the establishment of the database to July 31,2023.Two researchers independently screened the literature,extracted data,and assessed the risk of bias in the included studies.Meta-analysis was performed on the social alienation score of patients using Stata16.0 software,and descriptive analysis was performed on the influencing factors.Results Twenty-one studies involving five thousand and thirty-one cancer patients were included.Meta-analysis results showed that the social alienation score of cancer patients was 40.60[95%CI(38.61,42.58)].The influencing factors included demographic factors(age,gender,marital status)and other related factors(disease stage,social support).Conclusion The social alienation of cancer patients in China is above the average level,and further intervention and management are urgently needed.It is recommended to focus on female patients,patients with low education levels,low income,and tumor stageⅢor above,and formulate targeted intervention measures to improve the psychosocial problems of cancer patients and promote their active integration into society.
