腹腔镜微创手术治疗妊娠合并卵巢肿瘤患者术后恢复情况、妊娠结局及胎儿的影响

Postoperative Recovery,Pregnancy Outcome and Fetal Influence of Laparoscopic Minimally Invasive Surgery in the Treatment of Pregnancy Complicated with Ovarian Tumor

目的探讨腹腔镜微创手术治疗妊娠合并卵巢肿瘤患者术后恢复情况、妊娠结局及胎儿的影响。方法选取周口市妇幼保健院108例妊娠合并卵巢肿瘤患者,按随机数表法分为对照组54例行常规开腹手术,研究组54例行腹腔镜微创手术,随访1年,比较两组手术相关指标、妊娠结局、并发症及胎儿预后情况。结果与对照组相比,研究组手术时间、术后留置导尿管时间、术后排气时间、术后下床活动时间、住院时间更短,术中出血量更少;流产率、早产率、剖宫产率、胎儿窘迫率更少;并发症发生率更低(P均<0.05)。结论腹腔镜微创手术治疗妊娠合并卵巢肿瘤,可有效促进术后恢复,改善妊娠结局,降低术后并发症,不影响胎儿预后。

Objective To explore the effects of laparoscopic minimally invasive surgery on postoperative recovery,pregnancy outcome and fetus in pregnancy with ovarian tumors.Methods 108 patients with pregnancy combined with ovarian tumors were selected from Zhoukou Maternal and Child Health Hospital,and were divided into 54 cases in the control group,which underwent conventional open surgery,and 54 cases in the study group,which underwent minimally invasive laparoscopic surgery according to the method of random number table.The two groups were followed up for 1 year to compare the surgery-related indexes,pregnancy outcome,complications and fetal prognosis.Result Compared with the control group,the study group had shorter operation time,postoperative catheter time,postoperative exhaust time,postoperative ambulation time,hospital stay,less intraoperative bleeding,less abortion,premature delivery,cesarean section,fetal distress,and lower complication rate(all P<0.05).Apgar score of newborns in the study group,Geisel’s scores of each dimension were not significant(P>0.05).Conclusion Laparoscopic minimally invasive surgery for pregnancy combined with ovarian tumors can effectively promote postoperative recovery,improve pregnancy outcomes,reduce postoperative complications,and do not affect fetal outcomes.
